Assists in maintaining safety and comfort needs of patients and families. Assists with admission, transfer and discharge procedures per unit / service operations. Verbally and non-verbally communicates responsiveness and support to patients, families, and other staff. Observes general behavior and reports same to the registered nurse to whom assigned. Performs safety observations for patients on continuous observation or in restraints, per policy and as directed. Provides assistance to patients in the activities as applicable. Completes delegated responsibilities as directed by the registered nurse. Uses a clear, caring and respectful communication style with patients and families, nurses and other members of the health care team. Functions as a respectful member of the health care team. Answers patients call lights promptly and effectively; completes patient requests for assistance with personal care and activity, and reports those requests not within the role of the patient care technician to the professional nurse assigned to the patient. Reports all observations concerning patients to the professional nurse responsible for the patient on a given shift Responds to requests for specific tasks as delegated by any registered professional nurse. Provides for security of patients personal property, and handles valuables according to hospital procedure. Maintains cleanliness of the nursing unit as appropriate to position. (i.e., utility room, unit refrigerators, etc.). Gives direct assistance to all team members, including registered nurses, licensed practical nurses, physicians, nursing attendants, patient unit clerks, escort, dietary and other health care employees. Assists in having patient ready for scheduled appointments including placing the patient on a stretcher or in a wheelchair. Runs errands to other departments, including but not limited to, the delivery of specimens and equipment. Assists in keeping patient environment, patient equipment and patient care areas/units clean, safe and orderly. Ensures that necessary equipment and supplies are present in the patient rooms and on the unit. Minimum Qualifications: To qualify you must have a Patient Care Technician Minimum Qualifications: High School graduate or equivalent required. Graduate of a PCT program w/ at least 6 months hospital experience OR CNA graduate plus course completion in EKG & Phlebotomy and PCT certification eligible plus at least 6 months hospital experience, OR PCT Certification through National Healthcareer Association (NHA) w/ at least 6 months hospital experience. BLS or Heartsaver CPR/AED certification. (Issued by the American Heart Association-AHA) OR Heartsaver CPR/AED certification (Issued by the American Heart Association-AHA) is required upon entry to the position Must complete PCT online program within 3 months of hire. Senior Nursing Attendant Minimum Qualifications: High School Diploma or equivalent Required. The QM Coordinator will collaborate with key stakeholders to improve care processes by performing quality medical record reviews, assisting in the improvement of the clinical operations, maintaining accurate records of review activities, developing formal charters and improvement plans. They will also be responsible for maintaining up-to-date knowledge of regulatory standards and guidelines set forth by accrediting bodies. In collaboration with Leadership, Nursing, Medical Staff and other ancillary departments, they will prepare for, participate in, and follow up on regulatory, accreditation, and certification surveys and activities to facilitate continuous survey readiness. Serving as both a functional leader and data expert in a variety of tasks, namely; quality data reporting programs and measures, data compilation and analysis, process evaluation, and ongoing creation and maintenance of metric dashboards/displays of data to medical, clinical, and executive leadership and other committees as assigned. A Brief Overview The Care Transitions Coordinator is responsible for facilitating and coordinating services for patients through the continuum of care. The Care Transitions Coordinator ensures appropriate utilization of resources and safe delivery of care to all patients and provides quality and cost-effective care that will lower Length of Stay (LOS) and decrease the fragmentation of services that can potentially compromise patient care. The Care Transitions Coordinator play a pivotal role for patients as part of the interdisciplinary team in order to ensure patient safety, quality of care and positive patient outcomes. Rotation of coverage for weekends and holidays will be required and are assigned throughout the year. What You Will Do Modification of plan of care, as necessary, to meet the ongoing needs of the patient Communicate relevant information to third party payors and the care team. Completion of all required documentation in patient records Issues Notices of Non-Coverage per hospital policy. Refers appropriate cases for social work intervention based on department criteria Knowledgeable about Indigent Medication Programs Verifies patient demographics and insurance information is correct Expedites execution of plan of care for patients in Observation units. Arranges transportation for patients slated for discharge. Uphold compliance of regulatory standards for Observation patients including CMS requirement for Code 44. Utilizes “Patient Choice” list for support services post discharge and documents. Initiates home care referrals for VNS, orders durable medical equipment and arranges ambulance transportation as needed for patients prior to discharge or transfer to another facility. Participates as an active, informative and cooperative team member of the health care staff by attending discharge rounds for case finding/follow up and participating in the discharge plan while acting as an advocate for the patient. Interprets and reports responses of patients/clients to appropriate members of the health team. Participates with interdisciplinary health team members to plan a comprehensive patient care program. Determines and assigns those components of the nursing regimen which may be carried out by allied nursing personnel. Provides teaching, guidance and direction to allied nursing personnel in rendering patient care. Participates in defining, implementing, maintaining and interpreting standards of nursing practice. Participates in departmental and interdisciplinary conferences pertaining to policies and procedures affecting nursing practice. Participates in determining conditions, resources and policies essential to the delivery of nursing care services. Participates in development and maintenance of a system of evaluation of nursing care, practices and procedures, including evaluation of nursing personnel. Interprets philosophy and objectives of agency and Nursing Department to patients/clients, families and other groups. Maintains professional competence through participation in continuing education and other appropriate learning experiences. Performs other professional nursing activities as delegated by nursing service administration. Minimum Qualifications A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA). Special Note For assignments in Post-Acute Care ONLY, individuals meeting the following criteria may also be considered for appointment: Graduation from a nursing education program registered by the New York State Education Department (NYSED) as licensure qualifying for Registered Professional Nurse (RN) and has applied for licensure as an RN. These unlicensed individuals may be employed to practice under the immediate and personal supervision of a NYS licensed, currently registered, RN for up to 90 days immediately following graduation pending receipt of a limited permit, which selected candidates must apply for and receive within the same 90-day period. Once a limited permit is received, individuals can practice nursing, under the immediate and personal supervision of a NYS licensed, currently registered, RN at a NYC Health + Hospitals health care setting for up to one (1) year from the date of issuance of the limited permit. In addition, must hold or obtain through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA).
