Description The Nurse Manager is responsible for the management of nursing practice and operations on a designated unit/service. This includes, but is not limited to, the management of: clinical nursing practice, staff recruitment, retention, labor relations, supply chain and budget, staffing, employee performance evaluation, quality improvement, staff education, medical records, internal and external regulatory and survey requirements. The Nurse Manager collaborates with multiple professionals to support and coordinate the provision / management of patient care. He or she works closely with the Nursing Clinical Director, unit Dyad partner and staff in insuring that the Service/Unit maintains the highest standards of patient care and is responsive to the needs of the patient & family as well as the organization. Responsibilities PATIENT CARE 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of the patient experience. 2. Demonstrates proficiency in all aspects of Purposeful Hourly Rounding (PHR) and role models expected behaviors. 3. Ensures the integration of PHR into clinical activities to enhance Relationship Centered Care. 4. Validates staff members round hourly on all patients by direct observation and patient feedback. 5. Assesses staff competency in PHR and provides consistent meaningful feedback with more focused feedback as needed. 6. Conducts rounding of all patients on a daily basis (Monday-Friday.) 7. Communicates quantitative and qualitative information regarding the patient experience and staff engagement to the staff and the Interdisciplinary Patient Care Team that leads to the development and implementation of an action plan. 8. Collaborates with the physician dyad and escalates concerns as appropriate. 9. Follows up on all patient concerns in a timely manner and provides service recovery as needed. 10. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. PATIENT EXPERIENCE 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions but not limited to: Admission Welcome, Bedside Shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding (PHR), Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Huddles with staff at the start of each shift, identifying high-risk patients who require special attention. 3. Communicates to nursing leadership concerns or issues identified during huddle and throughout shift. 4. Initiates service recovery for patients and family members who have concerns and escalates to Associate Director of Nursing/designee and Patient Relations as needed. 5. Interprets and communicates patient experience scores and benchmarks and keeps staff informed of progress and areas of opportunity. 6. Considers the patient’s values, preferences, cultural diversity, expressed needs and knowledge in all aspects of care. 7. Empowers staff to use evidence-based practices to increase understanding of patients’ perceptions of care. 8. Demonstrates caring, respect, compassion, empathy and active listening through verbal and body language and actions. QUALITY AND SAFETY 1. Designs, supports and leads a safe, therapeutic and efficient patient-centered care environment for professional practice. 2. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and implements corrective action plans. 3. Fosters a fair and open culture that encourages error and near-miss occurrence reporting. 4. Leads interdisciplinary quality improvement teams using the principles of high reliability. 5. Reviews unit progress, changes, and compliance with quality and safety metrics with clinical nurses and other members of the healthcare team. 6. Responds to escalations of potential safety hazards or gaps from best practice by correcting concerns following institutional chain of command protocol. 7. Monitors institutional information technology infrastructure and corrects gaps in clinical nurse documentation. 8. Leads staff participation in quality and safety initiatives and mandated compliance measures. 9. Demonstrates accountability for all unit quality and safety practices and compliance, and ensures required evaluation of staff competencies is maintained. OPERATIONS AND ADMINISTRATION 1. Supports implementation of the mission, vision, goals and objectives of the department, throughout the clinical service and individual patient care units. 2. Provides administrative and clinical oversight to ensure that all activity (staffing, support services, inter disciplinary collaboration, resources and financial viability) supports the goals of the organization. 3. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. 4. Maintains a nursing unit in compliance with all regulatory agency mandates, including the regulations of NYS DOH and The Joint Commission. 5. Oversees 24 hour patient care delivery, throughput, staffing and processes to ensure efficient and effective operations. 6. Prepares, implements and monitors unit budgets, including capital equipment allocation, budget variances, management of OT and vacancies according to MSHS financial goals. 7. Maintains control of established positions and manages the recruitment and retention of qualified staff. 8. Maintains patient/employee confidentiality in the management of information. 9. Evaluates nursing staff performance and mentors, coaches, counsels and disciplines staff as needed. 10. Monitors availability and maintenance of nursing equipment and supplies. 11. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. 12. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. 13. Facilitates organizational change initiatives by using effective strategies. 14. Leads a shared decision making model by establishing a structure of shared governance, just culture and staff engagement in practice councils, departmental meetings and initiatives. 15. Interprets and implements collective bargaining agreements affecting staff. 16. Responds to protest of assignment in a timely manner as detailed in the collective bargaining agreement and discusses the resolution in unit staff meetings. PROFESSIONAL DEVELOPMENT 1. Performs as a clinical leader who actively creates and supports nursing practice and an environment of care that reflects clinical excellence. 2. Organizes the environment of care to support clinical colleagues, patients and their families and members of the healthcare team. 3. Qualifications Bachelors in Nursing (required) and Masters required (enrolled in a program will be considered) 3-5 years nursing clinical care with Charge Nurse/Supervisory experience preferred. Registered Nurse 2 (T&RC) Positions perform duties in each of the three functional areas described below. Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Coordinates medical and nursing care needs with other health care providers and departments. Ensures proper functioning of equipment and unit compliance with safety and environmental standards. Monitors nursing care quality standards, and implements corrective actions as needed. Attends and participates in medical committees to represent nursing staff needs and the individual population, and participates in writing of care standards. Nursing Assistant 1: entry level; performs basic patient care duties under direction of a nurse, physician, or other clinician; may assist Nursing Assistants 2, physicians, and nurses in the performance of care functions. Incumbents at this level receive in-service training in a limited number of care procedures, leading to eligibility to qualify for appointment to Nursing Assistant 2. Nursing Assistant 2: full performance level; using detailed guidelines, standards, and procedures, and under the supervision of clinicians, observes patients and performs prescribed simple treatment procedures; attends to personal hygiene needs of patients; and performs limited cleaning and housekeeping duties directly related to patient care. Treatment tasks performed require only knowledge of a limited number of specific treatment procedures performed at the facility. Nursing Assistant (Certified): full performance level; performs duties similar to a Nursing Assistant 2, but functions are performed in a long-term care facility and require certification as a New York State Certified Nursing Assistant. Related Classes Rehabilitation Hospital Nursing Assistants perform similar functions to those of Nursing Assistants but are focused on rehabilitation activities and are only classified at DOH’s Helen Hayes Hospital. Licensed Practical Nurses are classified in a variety of State health care settings and observe patients for symptoms and reactions to treatments; treat patients for physical maladies as directed and under close supervision; and attend to patients’ personal hygiene needs. Incumbents are licensed New York State Licensed Practical Nurses. Illustrative Duties Nursing Assistant 1 Under Direct Clinical Supervision, Performs The Following Duties Changes simple dressings as directed, gives enemas, douches, and irrigations as directed. Applies abdominal “T” binders, hot water bottles, aqua “K” pads, ice bags, and coolers, and hot and cold compresses to patients as directed. Assists patients in collecting urine and stool specimens. Observes and records patients’ quantitative food and fluid intake and waste output. Takes temperature, pulse rate, blood pressure and respiration rate. Gives therapeutic baths such as sitz and fever sponge baths. Measures and records height and weight. Inspects functioning oxygen therapy equipment including tents, masks, and cannulae to ensure appropriate functioning. Recognizes and reports abnormal signs and symptoms of common diseases and conditions to clinical staff. Applies splints, braces, slings and other self-help devices. Operates a variety of specialized patient monitoring and treatment devices. Attends to patients’ hygiene and daily living needs in order to maintain an atmosphere conducive to patient comfort and treatment. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. Receives patients’ belongings and transports objects to safe storage, keeping appropriate records. Feeds patients requiring assistance. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. May perform a variety of limited housekeeping and cleaning duties related to direct patient care in order to ensure that treatment and patient areas are clean, sanitary, orderly, and properly supplied. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment. This information includes items such as observable physical behavioral changes, fatigue levels, and general behaviors and responses to routine treatments, such as foot soaks. Qualifications Nursing Assistant 1 Non-Competitive: possession of or ability to acquire the necessary knowledge, skills and abilities during the probationary period. Nursing Assistant 2 Non-Competitive: one year of satisfactory full-time experience in the provision of simple treatment and other patient care services under professional supervision.
