Job Summary JOB DESCRIPTION This is a Pre-Kidney Transplant nurse coordinator position. Under general direction and in collaboration with the multidisciplinary transplant team, the nurse coordinator provides expert nursing care for transplant patients, including candidates (pre-transplant), recipients (post-transplant) and/or living donors with a variety of complex treatment options. The nursing coordinator serves as a contact point for transplant patients, family members, referring physicians, staff as well as contact point for the coordination and facilitation of the care of the patient in specialty areas. The nurse coordinator provides leadership for collaborative and multidisciplinary patient care with emphasis on education and continuity of patient care for the entire period of care including inpatient, outpatient or both. The transplant nurse coordinator is responsible for monitoring and coordinating the care of transplant patients to ensure they reach their maximum health potential before and/or after transplant. Specific daily responsibilities include: triaging patient calls and lab results, coordinating studies and specialty referrals, confirming orders, providing ongoing transplant patient education, spearheading the coordination of care for the transplant patient across disciplines, and clinic work as needed. Pre-transplant coordinators will focus on the evaluation and workup of the pre transplant candidate. This position will support the transplant team by communicating with patients on behalf of providers. The nurse coordinator will respond to changes in patient condition as exhibited in signs, symptoms, and lab values in collaboration with the provider. The nurse will make changes to medication and treatment plans as delegated by the provider. The nurse coordinator will educate patient on any changes and document in the EHR. Responsibilities Pre and/or Post Transplant and Living Donor Coordinator: Coordinate the efforts of the multidisciplinary transplant team, consulting services and nursing staff on behalf of the transplant candidate, donor, or recipient. Provide transplant-specific nursing care for pre- and/or post- transplant patients who have a variety of complex medical issues. Provides ongoing patient education for the transplant patient and family. Serves as multidisciplinary contact point for all patient requests, studies, and other needs. On behalf of the patient, serves as primary point of contact for phone calls, requests, inquiries by patient, family members, and outside care providers. Facilitates labs, diagnostic studies, such as ultrasound, x-rays, interventional radiology procedures, nuclear medicine scans, and urology procedures. Obtain the results of studies done at other centers. Notifies provider of results as appropriate. Initiates MD consultations and referrals for the transplant patient as requested by provider. Assesses subjective and objective data and escalates appropriately. Reviews patient charts and medication profiles to confirm compliance to treatment regimen and protocols as established by transplant providers. Provides ongoing patient education of treatment plan. Communicate lab results and medication changes to patients by telephone under the direction of the transplant provider. Documents clearly and thoroughly; documentation includes but is not limited to patient records, requisitions, referrals and billing. Utilizes electronic medical record for documentation. Exhibit independence, self-direction and accountability to the patient, family and transplant team Pre Transplant Coordinator Only Participate in the multidisciplinary evaluation of the candidate. Present the candidate at transplant selection. Facilitate the multidisciplinary work-up of the waitlisted patient. Qualifications Required qualifications: Minimum of three (3) recent years of RN clinical experience in adult services, or an equivalent combination of education and relevant experience. Recent outpatient employment in the field of kidney transplantation or donation, Specialization may also include a year of RN employment in organ specific specialty (i.e. outpatient nephrology experience for kidney transplant, outpatient pulmonology experience for lung transplant etc.). Active Registered Nurse licensure in the State of California Active American Heart Association CPR certification Knowledge of end stage organ specific disease, and transplantation and immunosuppressive therapy. Understanding contraindications to transplant as well as common post-transplant complications. Under the direction and supervision of the Director of Nursing Services, the Treatment Nurse is responsible for all the treatments that are prescribed by the attending physician for all residents in the facility. QUALIFICATIONS • Graduate of an accredited school of professional nursing. • Current licensure as a Registered Nurse or a Licensed Vocational Nurse in the state in which practicing. • Possess one or more year’s experience in a skilled nursing care facility. • Ability to read, write and speak the English language. • Possess mental and physical health. GENERAL DUTIES AND RESPONSIBILITIES: CLINICAL • Assess resident’s skin condition by doing complete body assessment on admission, re-admission and return from day, night or extended leave of absence. • Initiate the Nurse’s Skin and Wound Progress report upon identification of any skin problem and/or pressure ulcer. • Notify attending physician promptly when any skin condition is identified. • Initiate all treatments immediately as prescribed by attending physician. • Ensure that all skin conditions have corresponding treatment orders. • Ensure that all sites of any skin problems are identified and documented on the nursing assessment form upon admission. • Photograph all newly admitted residents with any skin problems and with stage 2, 3, and 4 pressure ulcers. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator.
