A Brief Overview The Clinical Assistant Nurse Manager (CANM) is responsible for leading the shift operations within the clinical setting to ensure delivery of high-quality care, exceptional customer experience, and optimal patient flow. In conjunction with the Department Nursing Operations Manager and the Assistant Nursing Operations Manager (ANOM); the CANM provides guidance, fosters collaboration, and is responsible for the unit's clinical operations including patient care assignments, customer experience, productivity, and quality outcomes in settings where patients have an advanced illness or injury that routinely requires timely, intense, and complex care to stabilize and support the patient's medical condition. Manages staffing assignments for oncoming and off going shifts, based on patient acuity and skill set of team members to ensure patient care needs are met. Coordinates patient flow by working directly with the Administrative Supervisors, Care Coordinators, and Discharge Planners to achieve expected outcomes. Monitors overall status of patients within area of responsibility and closely monitors status of seriously ill patients. Coordinates and/or participates in the delivery of patient care ensuring compliance with physician orders and established policies, procedures, and standards of practice. Serves as a resource supporting frontline team members and providers as appropriate. Supports clinical collaboration focusing on expected patient length of stay. Performs direct patient care activities as required to meet operational needs. Interfaces with patients and families to enhance customer experience by conducting Nurse Leader rounds and investigating and responding to patient/family and physician complaints. Participates in and/or leads safety huddles. Embraces, communicates, and promotes change and problem solving. Assesses equipment needs to ensure staff have the needed equipment for patient care, and they can appropriately utilize equipment. Supports and sustains all leader-driven initiatives. Ensures the appropriate allocation/adjustment of staff, assignment of meal breaks, relief of team members as indicated, etc. Utilizes effective fiscal management skills and financial resources with decision-making. Collaborates with the in-home clinical and support staff team to effectively support the patient care needs. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned virtual unit/ department. Hospital Care at Home (Program Functions – In-home Nursing): Administers direct patient care within the hospital at home inpatient home setting where an acute care patient receives active treatment for an injury, episode of illness, a medical condition, or post intervention with assessment. Assesses the patient’s needs and contributes to the individualized plan of care based on patient needs and responses. Ability to travel and enter patient homes as the patient hospital at home and place of service within a specific geographical region. Collaborates with the virtual Patient Care Hub clinical team and support staff team to effectively deliver care to support the patient care needs. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit/ department. Competency to address any urgent and or emergent patient needs as directed. Demonstrates competency to work autonomously and be productive in a hospital at home environment. Capable of using in-home virtual technology to connect with the Patient Care Hub and conduct virtual visits for patients in the home Transports and delivers items, equipment, and medications (excluding controlled substances and IV chemotherapy drugs) as requested.
