Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. These units p rovide intermediate level care to patients with congenital and acquired heart disease as well as those with primary rhythm abnormalities. Units may also care for patients under local subspecialty care or general pediatrics. Nurses will interview and be hired to one of these two units depending on position availability at time of application. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area.
