Our Case Manager Registered Nurses (RN) are responsible for partnering with the Clinical Director and team to administer and coordinate skilled nursing care to patients requiring home care services. Services are provided in accordance with physician’s orders, under the direction and supervision of the Clinical Director, and in compliance with applicable laws and regulations and the policies of the organization. The Infusion Unit APP Specialist will work in a recently developed role embedded within our adult infusion unit to serve as an infusion unit first responder, promote infusion services access and efficiency, provide oncology urgent care/symptom management and provide generalist coverage within our ambulatory solid tumor disease centers or hematologic malignancies programs for anticipated leaves and absences. Join a large and supportive ambulatory oncology and infusion service line Advanced Practice Team in a role that specializes in the care of patients with a variety of medical oncology diagnoses. With an interdisciplinary team approach, this provider will promote a collaborative relationship between providers, professional nurses, research personnel and administrative staff, to provide safe, quality care in compliance with clinical trial protocols across the care continuum. APP Responsibilities: • While on the adult infusion unit, the APP oversees the care of patients with hematologic and oncologic disorders, serves as a first responder for acute reactions and medical emergencies, partners with primary disease center providers to care for complex oncology patients awaiting hospital admission and collaborates with the Cancer Institute weekend first responder team to provide access for urgent evaluation and symptom management during off-hours. • The NP/PA is also responsible for collaborative care of the patient with benign hematological disorders receiving care in the adult infusion unit. • While in the cancer institute ambulatory practices the APP Specialist is responsible for the care of oncology patients in the outpatient setting, specializing in either solid tumor malignancies or hematologic malignancies in partnership with primary disease center APPs, physicians, practices nurses and administrative support team members. • Flexibility in scheduling as this position may require weekend, holiday and evening coverage as well as variable weekday assignments. Overview Starting Rate: $46.50/hour The Residential Programs Nurse provides direct nursing oversight and management of medical and care components in accordance with Advocates and funder requirements. The Residential Program Nurse provides training, supervision, and oversight for both residents and staff in all aspects of medical and medication management. They will collaborate with the Nursing Director, Residential Program Directors and nursing team in the implementation and follow-through of the individualized service plans and documentation in accordance with the mission and regulations of the program. Minimum Education Required Bachelor's Degree Responsibilities Responsibilities include, but are not limited to: Take a lead role in assessing potential program participants at his or her residential site; identify the program participant’s needs and functional abilities. Coordinate assessment for new durable medical equipment needs. Provide oversite and ensure on- going equipment checks and regularly scheduled maintenance. Participate initially and at least annually in a team assessment with the applicant, family member / guardian, and Residential Program Director as to whether the applicant can manage their personal care services.
