MDs, RNs, unit secretaries, and ancillary staff work collaboratively to care for a wide variety of patient populations. Interdisciplinary rounds are performed twice daily. The complexity of these patients requires a patient ratio of 1:1 or 1:2. The MICU population may at any given time consist of a diverse range of morbidities including, medical patients diagnosed with respiratory failure requiring mechanical ventilation, multi-system organ failure/ sepsis, renal failure, gastrointestinal bleeding, pulmonary hypertension, and liver failure. Interventions, depending upon the disease process, may require continuous renal replacement therapy (CRRT), hemodynamic drips, and inhaled prostacycline. Nurses operate in collaboration with other staff as well as autonomously when as appropriate, evidenced by nurse driven insulin protocol for DKA patients. Job Description The PICU is seeking a Registered Nurse to join our team in Wilmington, DE. The Pediatric Intensive Care Unit of Nemours Children's Health, Delaware is a 24-bed unit in a Level 1 Trauma Center. Patients requiring care and surveillance in the PICU will have conditions and diagnoses which may include, but are not limited to, those producing: acute and reversible loss of consciousness, respiratory failure, compromised air exchange, cardiovascular dysfunction, hepatic failure, hemodynamic instability, renal dysfunction, central nervous system injury or dysfunction, acute trauma and organ failure requiring transplantation. Treatments or interventions performed in the PICU include measures utilizing advanced life support technology. These include, but are not limited to cardiopulmonary resuscitation, tracheal intubation, ECMO, pharmacologic support of circulation, mechanical ventilation, point of care monitoring, CRRT, thoracentesis, and hemodynamic, intracranial pressure, intravascular monitoring. Each patient room has electrocardiographic, pulse oximetry, respiratory, hemodynamic and intracranial pressure monitoring capability, as well as central EKG and respiratory monitoring. Portable monitoring during patient transport is provided. Special equipment includes radiant warmer beds, heating/cooling blanket systems, Mapleson manual ventilation system, end tidal CO2 monitors, ventilators, hemodynamic monitoring systems, intracranial pressure monitoring system, ventriculostomy systems, infusion pumps, blood warmer, continuous renal replacement systems, and peripheral nerve stimulators. Requirements Be a graduate of an accredited school of nursing; BSN preferred. Experienced pediatric intensive care nurse of at least 2 years preferred. The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. The Registered Nurse Assessment Coordinator is responsible for the timely completion of all OBRA and Medicare assessments to comply with federal regulations. Oversees the overall process and tracking of Prospective Payment System (PPS) Minimum Data Set (MDS) assessments including completion, submission and correction as needed. Job Responsibilities: Integrates nursing, dietitian, therapeutic recreation, restorative, rehabilitation, and physician services to ensure accurate assessments which capture all clinical conditions and provided services. Confirms payer source of all residents per HIQA. Ensures the accurate and timely completion of all MDS Assessments including OBRA and Medicare PPS. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for electronic submission of all MDS assessments. Verifies electronic submission of MDS and performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness of MDS and thoroughness of documentation as mandated by federal and state standards. Schedules resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference Assists disciplines with care planning process by ensuring the MDS information is completed timely, the Care Area Assessments (CAAs) are analyzed thoroughly, and the team has completed the MDS prior to care conference. Ensures all Medicare documentation follows federal and state guidelines and is completed timely. This includes the physician certification/Recertification, Traditional Medicare and Medicare Advantage non-coverage letters for part A and B. Assess and evaluates the outcomes of the RAI process to determine additional training, education and monitoring needs Validates that the documentation supports MDS coding. Collaborates with the RAI Specialist(s) and the Interdisciplinary team to identify and capture opportunities to assist residents in reaching their optimal physical and psychosocial levels. Job Qualifications: Education: Graduate of accredited school of nursing. Certificate/License: Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in PA. Work Experience: One year of clinical experience. Experience with MDS completion, reimbursement, clinical resource utilization is highly desirable.
