Job Responsibility Maintains current knowledge of and ensures compliance with all regulatory and professional standards of care which includes Joint Commission on Accreditation of Healthcare Organizations (JCAHO), Centers for Medicare and Medicaid Services (CMS), International Health Institute (IHI), Community Alternatives Programs (CAP), etc. Develops, prepares and monitors the personnel, long-term unit staffing schedules, supply and equipment budgets in collaboration with the staff and support department directors in accordance with established staffing needs. Identifies research protocols effecting the patient population served and keeps nursing staff informed. Develops unit goals and objectives to support the institutional values and goals, with input from staff. Responsible for the overall planning process for the unit. Conducts monthly meetings with the staff to address equipment, scheduling, goals and objectives, performance metrics and improvement activities, and any staff concerns. Makes recommendations and provides opportunities for growth and development of personnel and arranges for appropriate education and training. Also identify at least one potential successor to the Nurse Manager role. Completes appropriate management conferences. Oversees medication management, administration, procurement (from hospital and VFC) and storage. Performs related duties as required. Job Qualification Bachelor of Science in Nursing required. MDS Coordinator - SNF Located in The Bronx, NY Salary: $125K to $165K range (based on experience) Responsibilities of the MDS Coordinator-Nursing Home: Completes assessments, Minimum Data Set (MDS) and care plans for all residents assigned Monitors completion of MDSs by other disciplines within timeframes prescribed by regulatory guidelines Advises supervisor of incomplete and/or untimely assessments by disciplines other than nursing. Ensures accurate, timely completion of the MDS/RAPs/Triggers sheet for assigned residents. Initiates care plans and supporting activities that will result in best possible outcome for assigned residents. Generates and distributes monthly care plan calendar for the following month. Registered Nurse ACF Float- Adult Care Facilities Open Locations: Manhattan, NY, Bronx, NY, Brooklyn, NY, Queens, NY, and Staten Island, NY Salary: $120K to $130K range; Based on experience Qualifications of the RN Float: Must have current New York RN license Must have hospital experience Candidate must be able to commute to multiple facilities Excellent clinical and communication skills Responsibilities of the RN Float: Performs a home care assessment to determine patient’s eligibility for services. If not eligible/appropriate for home care services COC will indicate not admitted in the system and reason for the determination.
