Description Summary: Accountable for providing advanced nursing and consultation services in accordance with written standardized procedures, the APRN manages the primary care needs of InterCommunity’s patient population. Essential Duties & Responsibilities: Provides appropriate nursing and/or health care services to patients including obtaining complete histories; performing pertinent physical exams with assessment of normal and abnormal findings on new patients; clinical assessments, treatments, diagnosing medical conditions, assessing emergencies, ordering diagnostic studies, prescribing medications Obtains interval histories and performs pertinent examinations on return patients Performs various procedures possibly including, but not limited to: Administering injections Performing and/or assisting with minor surgical procedures Obtaining PAP smears, genital, wound and other cultures as indicated Obtaining EKG tracings Performing phlebotomy Irrigating, staining eyes for foreign bodies, abrasions Irrigating ear canals, and removing cerumen Performs specialized duties in providing medical care to a variety of patients including individuals with acute and chronic conditions Consults with clinical staff regarding assessment methods and treatment plans Assists in determining need for contractual health services Provides direction to staff on appropriate health record documentation Prepares reports on health services, problems and investigations Participates in quality assurance and other clinical committees as needed May assist families and other groups in understanding medical needs of patients serviced by agency; may participate as a member of multi-disciplinary evaluation team Recognizes situations which require the immediate attention of a physician and initiates life-saving procedures when necessary. This role involves regular physical activity, including lifting patients over 200 lbs with assistance, transporting supplies and equipment up to 20 lbs for distances of 500 feet, and frequent standing, walking, sitting, stooping, and reaching. Responsibilities: Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and the Interdisciplinary Group (IDG) to develop, implement, and evaluate individualized plans of care in alignment with regulatory requirements and the Case Management Recipe. Maintains timely and accurate documentation, ensures care plans reflect current patient status, and communicates effectively with the care team to coordinate optimal use of resources, especially in complex or high-risk situations. Provides education and emotional support to patients and caregivers based on individual needs, and actively participates in on-call rotations, admissions, and care transitions as directed. If serving as a Care Manager, coordinates all aspects of patient care including scheduling, medication management, plan of care oversight, IDG collaboration, and supervision of LPNs, LVNs, and Hospice Aides; participates in IDG meetings and patient/family conferences. Lead, coach, mentor, and develop nursing staff while promoting engagement, accountability, and professional growth. Partner with physicians and interdisciplinary teams to promote effective patient care, communication, and patient flow. Monitor quality, safety, regulatory compliance, and patient care outcomes. Support recruitment, onboarding, retention, competency development, and performance management. Manage departmental resources responsibly and promote fiscal accountability. Identify and implement opportunities to improve patient care, efficiency, patient experience, and department performance.
