Summary: The Penn Medicine at Home (PMAH) Home Health Aide is responsible for implementing a Plan of Care, assisting the patient/family with performing personal care tasks, and assisting with the activities of daily living. The individual is responsible for performing home health or hospice visits throughout a designated service area. A Home Health Aide is responsible for completing timely and accurate submission of patient documentation. Responsibilities: Patient Care – Directly performs or assists with bathing, oral hygiene, skin and hair care of the patient. Prepares light meals for patients and assists with eating. Assists with the clothing of patient. Assists the patient with use of a bedpan, commode, and urinal. Empty Foley catheter drainage bag and records amount on weekly Home Health Aide weekly report when applicable. Exercise and Transfers- Provides assistance with home exercise programs to an assigned group of patients. Assists with patient’s ambulatory and with bed to chair transfer. Turns patients in bed to positions of comfort. Patient Environment - Performs household and other environmental services tasks specified in the Home Health Aide Plan that are essential to entity. Assists with pre-poured medications, which are ordinarily self-administered. Provides respite care to relieve primary caregiver as assigned. Documentation - Accurately documents care provided to patients on a timely basis according to Medicare/JCAHO standards. Completes allotment of hours as ordered by department and communicates changes in schedule. Documents and verifies with signature all Home Health Aide Activity Sheets and obtain caregiver or authorized facility signature. Oversees the timely submission of the ‘Rap Check Report’ to support government reimbursement. Communication - Communicates any unusual findings and/or occurrences, obvious changes in patient’s physical, mental or social status, home safety hazards, refusal of care, or patient/family dissatisfaction to the primary nurse and/or Home Health Aide Supervisor. Promotes easy communication between the family, RN Case Manager and Scheduling Coordinator. Plan of Care - Follows the written care plan in relation to days of week and number of hours assigned for patient visits. Provides care only as outlined in the Plan of Care unless instructed by Case Manager of designee. Reviews biweekly IDT Plan of Care before providing care to patients. Employee Travel - Employee must provide their own transportation to and from patient’s homes and meet current state requirements for operating a motor vehicle. Education - Attends annual in-services and completes a minimum of 12 hours of continuing education annually. Demonstrates understanding and adherence to PMAH procedures. Meets all competencies as defined for Home Health Aides. Credentials: Car Insurance (Required) Proof of automobile insurance (Required) Certified Nurse Aid (Required) Certification as a Nursing Assistant (in PA) or Home Care Aide (in NJ) depending on geographic location and scope of responsibilities or student nursing assistant in lieu of certification, matriculated in an accredited BSN nursing program having minimally completed the nursing foundations/fundamentals clinical course with a 3.0 GPA overall and in the nursing major. Nursing Students are required to submit grade reports/transcripts to their supervisor/Regulatory Compliance within 60 days of their course completion. Nursing students employed as HHAs, must immediately (within 24 hours) and in writing report to their supervisor and Regulatory Compliance if they have not sustained the academic GPA requirement or discontinued/withdrawn from their matriculated BSN program. Failure to meet and maintain nursing student requirements in lieu of certification will result in termination of your nursing assistant position. As a Full-Time Registered Nurse, Regional Clinical Manager, you’ll serve as a client-facing clinical leader overseeing clinical operations across our New Brunswick, Raritan, and Titusville, NJ, and Jacksonville, FL, Health Center locations. You’ll also provide care to client employees at our Health Center in New Brunswick, NJ , while partnering with a strong, collaborative team dedicated to delivering exceptional, patient-centered care. This position is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m. What You'll Do Oversees the management of single or multiple (2+) health centers and lines of service across contract(s). Manages staff processes typically composed of health center manager, and multiple technical Team Leads as well as indirectly managing all daily operational processes for Medical Directors and clinical providers. Assists in the creation of site-specific goals and measurements related to the needs of the client(s), including management of metrics to Performance Guarantees. Effectively communicates across the various sites and to stakeholders to ensure consistency in messaging. For multiple contract oversight, effectively communicates with DCO to ensure site level priorities are addressed and messaging is consistent across the book of business (BOB). Compiles input and contributes to performance appraisal process for all indirect reports. Responsible for following appropriate processes for hiring, staff development and performance management and provides input to AVP and DCO as needed. Responsible for interviewing, training, and the successful orientation and onboarding of new hires as applicable. Manages daily operations, workflow, work schedules, and timecard monitoring, to ensure efficient and effective health center performance. Identifies process improvement opportunities, workflow efficiencies, determines root cause analysis, creates action plans and presents resolutions and recommendations to the AVP and DCO. Makes recommendations regarding staffing model based on objective scheduling & volume analysis – presents to AVP and DCOs for approval. Understands and complies with all regulatory, procedural and licensing requirements. Oversees the execution of site level quality program and is responsible for supporting and oversight of AAAHC standards across sites. Provides oversight on Member Engagement Site Action plans and outcome reporting. Develops action plans around patient utilization and penetration. Completes reports as needed and provides assistance to Clinical Quality and Operations departments regarding investigations and the resolution of issues or complaints. Coaches and provides feedback to staff on a regular basis. Manages internal budget, reviews expenses, purchases supplies, inventories equipment, medication, lab tests etc. May serve as a client contact and provide reports and summaries of health center activities. Communicates and coordinates Corporate messages and ensures implementation of policies and procedures. Assists in the identification and use of per diem staff to ensure appropriate staffing levels are met. Works collaboratively with the health center managers and Medical Director at the site to manage internal medical staff site issues. Communicates regularly with staff, conducts meetings, and provides updates and shares information regularly. Acts as the health center’s infection control lead and collaborates with the corporate Director of Quality and Infection Control Officer. May provide patient care if qualified and where circumstance applies. May require other duties as assigned. What You'll Bring Bachelor’s degree or equivalent work experience required. Current license as an RN in practicing state. Certification in Occupational Health (COHN/COHN-S) may be required for some sites. Current hands-on certification in AHA or ARC Basic Life Support for health care providers is required. Minimum 3+ years’ experience in the medical field. 2 – 3 years’ management experience. Prior management experience managing a large group a must. Experience accessing CDC, WHO, APIC or other industry standards for Infection Prevention practices.
