Position Responsibilities: · Assists in the preparation of the unit and patient rooms for new admissions, patient transfers, and post-operative care. Ensures each room is clean, organized, and fully stocked with necessary supplies. · Uses proper body mechanics when positioning lifting turning and ambulating patients. Applies and uses appropriate equipment as needed. · Understands and implements the measures used to reduce the risk of health associated infections. · Reports any changes observed in the condition or behavior of the patient to the LPN/Registered Nurse in a timely manner. · Assists with the admission transport transfer and discharge of patients including the care of personal property and clothing. Lists the belongings flow sheet. Labels belongings bed and hospital equipment with patient identification. Orients newly admitted patients to surroundings. Receives and acknowledges patients independent of assignment. Assures collection of all belongings at the time of transfer/discharge. · Demonstrates ability to provide safe care to patients. · Ensures patient personal and oral hygiene needs are met daily and/or as necessary. Provides catheters care daily and/or as needed. Reports any pertinent observations to LPN/RN. · Accurately takes vital signs and records in patient record promptly. Reports to LPN/RN promptly. · Distribute water records correct intake and output when indicated within shift. · Obtains and labels specimens accurately I.e. sputum stool urine maintaining precautions at all times. · Prepares patients for surgery and/or other procedures including prep and clip vital sign taking and recording assisting patient with bathing and/or voiding and removing and securing dentures jewelry and other belongings. · Weigh patients as assigned; records weights and alerts RN to any weight gain/loss. · Makes frequent patient rounds at least every two (2) hours. Repositions for comfort and offers bathroom assistance. · Assists RN with post-mortem care including labeling and securing of all patient valuables. · Provides patient care as directed by licensed nurse to include but not limited to: enemas sitz baths applying and removing ice bags k-pads binders and/or pelvic/buck's traction blood glucose monitoring and simple dress changes. · All other duties as assigned. Position Responsibilities: · Successfully challenges EKG test upon hire, if previously trained or successfully completes EKG program within 90 days of hire. · Successfully interprets life-threatening dysrhythmia and subsequent treatment. · Demonstrates safe and correct defibrillation. · Administers anti-dysrhythmic therapy as ordered. · Demonstrates basic understanding of patient’s daily status, including reason for admission, surgeries/procedures performed, pertinent diagnostic data, learning needs of the patient, and discharge plan. · Demonstrates safe and correct care of the patient with internal cardiac devices. · Demonstrates understanding of patient multi-system complications, evidence by identification and reporting of abnormal findings to appropriate physician in a timely manner and implementation of nursing. · Monitors hemodynamically unstable patients, assesses, reports and intervenes appropriately. · Performs initial vital signs and admission history to include introduction to patient and family. Assessment of chief complaint and notification of physician within 15 minutes of arrival to unit. · Obtains and documents a complete nursing admission assessment, including physiological and psychological factors to develop an interdisciplinary plan of care within eight hours of admission. · Performs and documents an accurate systems assessment at the beginning of each shift. · Responds to diagnostic data and initiates appropriate action or intervention. · Assesses patient education needs on admission and daily. · Assesses psychosocial needs specific to patient's age, growth, and development. · Initiates an interdisciplinary plan of care within eight hours of admission. · Establishes priorities for patient care based on patient needs as evidenced by completion of assigned duties within shift. · Verbalizes understanding of patient diagnosis and interdisciplinary plan of care. · Safely and correctly initiates and documents nursing interventions in emergency situations. · Performs 24-hour chart check at the beginning of each shift. · Administers, observes, and documents intravenous therapy according to hospital policy. · Administers medications within 30 minutes of scheduled time. Observes and documents effectiveness, recognizes and reports adverse reactions immediately. · Provides individualized instructions each shift to patient/significant other regarding the plan of care. · Demonstrates safe and correct use of unit specific equipment. · Ensures that each patient's general hygiene needs are met daily. · Performs pre-operative and post operative care and instruction for each assigned surgical patient during shift. · Adheres to standard precautions including hand hygiene guidelines at all times. · Documents specific patient valuables on designated form when received and immediately arranges for appropriate storage. · Performs dressing changes using aseptic technique. · Collects and labels specimens per policy when ordered by physician. · Performs bedside glucose monitoring according to policy. · Administers internal feedings via ng tube, g tube, and/or j tube. · Reports all abnormal findings immediately to physician. · Initiates wound and skin protocol upon identification of risk. · Performs ostomy care to include irrigation, appliance usage, and reinforcement of teaching. · Inserts intermittent or indwelling urinary catheter using aseptic technique. · Administers oxygen therapy according to physician order. · Transfers patients in/out of the unit in an efficient manner, by obtaining a complete and correct set of physician's orders. Completes documentation of patient transfer summary, and ensures appropriate completed referral forms accompany intra-facility transfers, including giving/receiving of report. · All duties as assigned. Overview The Registered Nurse (RN) provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse. The RN is expected to perform all staff nurse activities and function as charge nurse when assigned.
