Overview The Registered Nurse (RN) provides nursing care to patients utilizing the nursing process and standards of practice for a professional nurse. The RN is expected to perform all staff nurse activities and function as charge nurse when assigned. The RN maintains overall accountability/responsibility for assigned patients, even though some activities/tasks may be delegated to Technical Nurses, Patient Care Associates, and/or Sitters. Assignments are performed appropriately to age and developmental level of patients being served. Qualifications Age of Patients Served: All Age Groups. Required Education: Associate's Degree Specific course work: Graduate of an accredited school of nursing. New hires effective June 14, 2017, must have a BSN degree from an accredited school of nursing within three (3) years of hire or transfer into the position. New hires or transfers into in the position between August 1, 2013 and June 13, 2017 must abide with the conditions in his or her offer letter and the verbiage in the job description signed upon hire or transfer into the position. Current employees who were hired or transferred into the position prior to August 1, 2013 will be grandfathered. Preferred Education: BSN Necessary Skills: 1. Demonstrates teamwork and communicates in a professional manner with all members of the healthcare team. 2. Demonstrates the ability to learn and apply computer technologies and procedures that affect patient care delivery. Required Licensure/Certifications: Licensed to practice nursing in the state of Florida or confirmation letter establishing eligibility for issuance upon receipt of SSN. BLS from the American Heart Association or American Red Cross on the first day of employment is required for all areas. ACLS from the American Heart Association is required at hire or transfer for the following departments: Cardiac Holding Area (730501), CVICU (730400), MICU (700400), PICU (660401), North Pre Op/PACU (633680) and Trauma Center (713100). ACLS from the American Heart Association is required within six (6) months of hire or transfer for the following departments: 3 ICU-North (620480), 5N CCU (730301), Cardiac Rehab (733100), Non Invasive Cardiology North (733080), and Noninvasive Cardiac Lab (733000). ACLS from the American Heart Association is required within six (6) months of hire or transfer for Progressive Registered Nurses in Intensive Care Units. PALS from the American Heart Association is required within six (6) months of hire or transfer for the following departments: PICU (660401) and Trauma Center (713100). Fellowship RN's must obtain their ACLS, PALS and NRP within six (6) months of hire or transfer if required by the department. Crisis Prevention Institute training within ninety (90) days, and then annually, is required for Psych Unit. Preferred Licensure/Certifications: Professional certification preferred. Required Experience: Less than one year of related experience. Preferred Experience: One (1) year of clinical experience.

Purpose of Your Job Position The primary purpose of your position is to conduct and coordinate the development and completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility. Delegation of Authority As MDS Coordinator you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. Ensure assessments accurately reflect the physical, mental and psychosocial status each resident. Ensure assessments reflect optimal reimbursement for services provided. Ensure appropriate documentation to support services provided. Initiates, directs and maintains the Medicare PPS and OBRA assessment schedules to ensure timely completion of all assessments. Complete entry trackers and discharge assessments accurately and timely. Ensure that an Initial Admission Assessment is completed within fourteen (14) days of the resident’s admission. Ensure that quarterly and annual resident assessments are completed on a timely basis. Collaborate with the IDC team to identify significant change in resident status and, ensures that a comprehensive resident assessment is scheduled within fourteen (14) days of a significant change in the resident’s condition. Ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS. Sign and date the assessment instrument to certify its completion. Ensure that all assessments are completed and transmitted in a timely manner. Review validation reports for fatal errors and warnings and completes Modifications and Inactivations in accordance with CMS Correction Policy. Maintain a PPS pathway for all residents on Medicare Part A and Managed Care. Collaborate with facility Director of Rehabilitation to ensure the most appropriate ARD is utilized for Medicare assessments thru the use of PPS pathways Provide billers with PPS pathways to ensure accurate billing. Complete Comprehensive Status Updates to Case Managers on all managed care residents per company guidelines. Complete and maintain timely completion and signing of Physician Medicare Certifications. Prepare for State and Federal agency audits related to RAI process and reimbursement. Resident Rights Review complaints and grievances made by the resident and make a written and oral report to the Nurse Supervisor, LPN, and RN. Follow Facility’s established procedures. Maintain a written record of the resident’s complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint. Must adhere to all HIPAA requirements. Working Conditions Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses’ stations, resident rooms, etc.). ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Assisting in feeding the patient as assigned. Hospice Aides may not provide patients with homemade food items or baked goods that they themselves prepared 7. Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence. 8. Reporting any complaint of pain or other symptoms or changes in the patient's condition to the RN Case Manager. Also aware of the caregiver or other individuals living with the patient and interpersonal issues. 9. Assisting in ambulation and exercise as instructed by the hospice nurse or therapist. 10. Provide normal range of motion and transfers/positioning as directed. 11. Assistance with self-administered medications as allowed by state regulations, such as “reminders.” 12. Offering and assisting with bedpans and urinals. Providing assistance as assigned with light laundry needs, emptying waste baskets and other light homemaker tasks. 13. Performing range of motion and other simple procedures as ordered with appropriate completed/demonstrated skills competency. 14. Providing respite for patient's/family/caregiver when on-site, as appropriate. 15. Keeping patient's living area clean and orderly, as assigned. 16. Adhering to the organization's documentation and care procedures and standards of personal and professional conduct. 17. Participates in quality assessment performance improvement teams and activities. 18. Respectful of patient and family/caregiver environment and patient's personal needs. 19. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 20. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. JOB LIMITATIONS The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Nursing Assistant Certified (NAC) 3. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 4. Current Food Handlers Permit 5. High school diploma or General Equivalency Degree (GED) 6. Ability to read and follow written instructions and document care given. 7. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 8. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 9. Basic computer skills to document in electronic medical records (EMR) 10. At least 18 years of age. 11. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
