Job Overview Ebenezer Integrated Care & Rehab (EICR) serves seniors with long-term, transitional, and dementia care needs in the heart of downtown St. Paul. As part of Fairview's Community Health and Wellness Hub, EICR is committed to delivering innovative, collaborative care. The Fairview Community Health and Wellness Hub opened in 2022 to deliver a variety of services to the community—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. The Hub brings Fairview services together with local organizations—including Minnesota Community Care and Sanneh Foundation—to make it easier for people to access what they need to be well. We are seeking a Nursing Assistant (CNA/NAR) for our newest Skilled Nursing Community (Transitional Care units, Long Term Care) in downtown St. Paul, Minnesota! This role is responsible for providing resident care and services. This CNA/NAR schedule includes; 48 hours every two weeks ; Part Time Day Shift ; 6:00am - 2:30pm Block Schedule: Week 1: Monday, Tuesday, Saturday, and Sunday Week 2: Thursday and Friday Long Term Care Unit Every other weekend and holiday rotation Responsibilities Assists residents with personal cares based on individual plan of care: Provides AM/PM cares Provides full bath/shower weekly Provides oral hygiene per care plan Provides nail care, shaving as appropriate Checks hearing aids; cleans eyeglasses daily Assures that residents are clean, dry, and dressed appropriately Provides offloading for residents Performs Restorative Nursing Programs per facility policy. Provides adequate nutrition & hydration (distributes meal trays, assists residents as needed, documents intake/output meal consumption, provides nourishment & water between meals as ordered) Assures individual toileting plan is followed Answers call lights promptly Observes residents and reports status to appropriate staff: Receives report at the start of the shift Gives report to team leader at the end of the shift & PRN Notifies appropriate staff of any changes in a timely & appropriate manner (i.e. incidents, skin integrity, behavior changes, illness) Offers pertinent observations & input for documentation and care plan updates Utilizes Nursing Assistant Kardex or Care sheet Takes and documents weights as ordered Documents resident cares promptly in EHR Responds to emergency situations appropriately Handles resident and family complaints tactfully and refers concerns to appropriate staff members Follows Ebenezer policies and procedures Reports resident change of condition to Licensed Nurse Attends mandatory, annual, and any other educational opportunities on site. Ensures assessments are completed every 90 days per regulatory requirements. Works with resident and/or responsible party through collaboration, education, and timely communication to ensure appropriate service and placement. Service Agreements: assure that resident has an up to date service agreement at time of admission, with any changes in services and annually.
