Job Description Summary: An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physician’s plan of care. Works under structured supervision of the designated supervisor. Job Description: Job Responsibilities (listed in order of importance and/or time spent) Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals. Organizes and participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status or physician orders to achieve established or revised patient care goals. Assesses patient needs and physical status at each skilled visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order. Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance. Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule. Responds promptly and appropriately to patient requests. Initiates emergency procedures as necessary. Accepts accountability for own practice through ethical and professional conduct. Follows established programs and practice within policies and procedures reflective of Option Care’s mission, values, and objectives. Observes legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Option Care information including adherence to HIPAA regulations. Effectively provides oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned. Physical Demand Requirements Ability to lift up to 50 pounds with a maximum lifting of 75 pounds. Exerting up to 20 pounds of force occasionally, or up to 10 pounds of force frequently. Physical demands may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying. Fine motor skills and visual acuity required by this job include ability to see up close and from a distance, color and peripheral vision, depth perception and the ability to adjust focus. RN MDS Coordinator Shift: 8:00am-4:30pm M-F The MDS Coordinator is responsible for the timely and accurate completion of the Federal and State assessment tools but may also be called upon to perform duties of an RN. This position requires an understanding of the MDS rules and regulations as described in the MDS User's Manual, including item coding, RAPS, CAA's, Care Planning, electronic submission, scheduling, PPS including knowledge of MDS submission process, final validation reports, and an understanding of Medicare rules and regulations including coverage, benefit periods, certification, 30 Day rule. Essential Duties Maintains Tracking Tools for the MDS and Medicare. Makes skilled services decisions and initiates Medicare non-coverage letters and expedited determination notices. Provides nursing care, as needed, in accordance with Resident Care Policies and Procedures and maintains the safety and well-being of the residents. Acts appropriately under the direction of the assigned supervisor and acts as an active member of the interdisciplinary team (IDT). Attends meeting of supervisory and administrative staff, as applicable. Participates actively on the Quality Assurance Team (QA). Completes assigned sections of MDS assessments for nursing. Reviews the health records for each resident, including reviewing written notations of significant changes affecting: Level of activity Eating habits Physical, mental, and emotional status Participates in planning and adjustment of Medicare assessment scheduling. Coordinates the IDT to accomplish timely completion of assignments, Care Plans, and CAA summaries. Assists and teaches the Care Planning process to nurses and other members of the team including ADL training. Communicates with nursing and clinical personnel on the implementation of the resident Care Plan. Completes Medicare Daily Documentation Guide on admission. Assists with studies related to improvements of nursing care, if needed. Medication Aide Certified MAC Shifts Available: 7A-7P Full Time Performs resident care activities and related non-professional nursing services under the direction of the Charge Nurse/Nurse Manager to ensure quality care of residents. Essential Duties Provides individualized attention and nursing care in accordance with resident Care Plan which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating, etc. Responsible for observing and promptly reporting any changes in resident conditions to Charge Nurse/Nurse Manager (example: level of activity, eating habits, behaviors, and physical, mental and emotional status). Performs tasks assigned by Charge Nurse/Nurse Manager including vital signs, residents' weights, applying non-prescription creams/ointments, collecting specimens, etc. Escorts residents to dining room, assists with feeding, and documents meal consumption. Passes supplements, snacks, and fluids to residents per protocol and schedule and documents consumption. Provides care that maintains each resident's skin integrity in order to reduce the risk of pressure ulcers, skin tears, and other damage by changing incontinent residents per protocol, turning and repositioning immobile residents per protocol, and applying moisturizers to fragile skin as needed. Maintains clean, neat and organized work environment. Protects the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items to Charge Nurse/Nurse Manager per protocol. Answers resident call lights promptly and courteously. Assists with orienting residents and their families to the nursing home upon admission. Escorts residents to appointments, including beauty shop, activities and church services and participates in activities and functions as directed. MAC Qualifications Valid MAC certification in the state of Alabama. As a Unit Manager RN, you will play a pivotal role in overseeing the nursing staff and ensuring the highest quality of care for our residents.
