Key Responsibilities: Client Assessments & Field Visits. Conduct in-home client assessments and gather detailed clinical and environmental information. Observe and document client condition, functional needs, and safety risks. Identify changes in condition and escalate appropriately. Care Plan & Clinical Support Prepare assessment documentation for RN review and approval. Assist with development and updates of care plans. Ensure all documentation is accurate, complete, and submitted in a timely manner.
