ACH is an implementation of one of three initial platform ventures and is a new model of high-acuity care for the home. ACH RNs work extensively within 'Medically Home' which has been chosen as Mayo Clinic Platform's partner to provide the technology that orchestrates the delivery of these services into the home and supporting infrastructure. Performs QA initiatives related to core Measures and Flagler Hospital Admission Standards.
