Job Requirements The Home Training Registered Nurse evaluates, trains, and provides support to the dialysis facility’s home patients, care partners, and families. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned.

Description The APN/NP is a specialty health care provider who provides medical and nursing services to individuals and families. The APN/NP is adept at diagnosing and managing acute and chronic illnesses, and emphasizes health promotion and disease prevention. Services include, but are not limited to, ordering, conducting and interpreting diagnostic and laboratory tests, prescribing medications, and teaching counseling individuals and families.
