JOB DESCRIPTION Job Summary Provides support for care management/care coordination long-term services and supports (LTSS)-specific activities. Collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum for members with high-need potential. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care. This position will be assisting members with Medicaid and Medicare benefits. This position may require home visits if needed for care plan review and signature. This position supports members that reside in Brooklyn, Queens, Bronx, Manhattan, Westchester and Long Island. Responsible for the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice. I. Major Responsibilities: 1. Position Qualifications: License/Certification/Education: Required: 1. Current Massachusetts Nursing licensure and registration. 2. BLS is a requirement for every RN unless advance cardiac life support (ACLS) certification is current, in addition to any unit specific certifications and competencies. Preferred : 1. Bachelor of Science degree in Nursing highly preferred. Experience/Skills: Required: 1. Thorough knowledge of the discipline of Nursing. Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements. Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents. III. Swan Hospice is looking for CHHA ( Certified Home Heath Aid ) with Hospice experience to join our team. We provide quality hospice care to patients in skilled nursing facilities, assisted living facilities and in private homes. We treat residents, families and loved ones with kindness, respect, compassion and dignity. We have a comprehensive team that can provide support and stability to residents and staff. Job Responsibilities Assists with the prescribed exercise and ROM activities which the resident/patient and Hospice aide have been taught by the Hospice Nurse. Provides assistance in toileting; positioning and transfer activities, frequent turning and positioning of a bed bound resident/patient. Provides or assists in ADL activities; personal hygiene, grooming, mouth, skin care and nails. Assists or reminds the patient to take medication, in compliance with regulations of appropriate regulatory and accrediting bodies. Assists with light housekeeping of individual's personal unit or room, including changing of bed linen, if needed and patient laundry. Feeds and assists with meals. Obtains temperature, pulse and respiration as supervised by the Hospice Nurse. Reports any changes in resident's/patient's condition to the Hospice Nurse and, if in a facility, to the facility nurse and documents the information , including the date and time of the report and the person receiving the information. Ideal candidates will have: HHA License in the state of NJ Understands the hospice philosophy Minimum of one year experience working in hospice Dependable transportation
