What you'll do The Nursing Assistant (NA) I is a key member to the nursing care team. Nutrition: Assist with meal tray delivery, collection, and documentation of amount consumed. Set up and assist patients with meals, including feeding. Performs patient data collection, including but not limited to: Use of data collection equipment: Vital signs, EKG, bladder scan, transfer devices, telemetry, Massimo Documentation in EMR: Components of vital signs, height and weight, intake and output measurement (including drains, urinary catheter and suction devices.) Essential care performed. Intentional rounding. Provides direct patient monitoring, including but not limited to: Safety Attendant (SA) at bedside for prevention of unintentional or intentional (suicide precautions) self-harm. Video surveillance. Performs patient care interventions as delegated and within scope of practice and validated competency, including but not limited to: Phlebotomy, EKG’s, specimen collection, blood glucose measurement and telemetry. Others patient care needs, including but not limited to: Prepare patient rooms for admissions, discharges. Update whiteboards. Provide unit / room stocking of supplies. Care of a patient with invasive lines and devices within NA scope of practice. Transporting patients on/off unit. Responding to emergency situation. Communication with patient care team to ensure delivery of quality care. What you'll do The primary purpose of the Licensed Practical Nurse, Charge is to evaluate and direct nursing care of residents on a particular nursing care unit. Integrate the philosophy and objectives of the Nursing Home and the Nursing Department into the management of nursing care. Initiates plan of care: Assign resident care in accordance with resident need and to the abilities of the staff members assigned to the unit. Observes the residents physiological and psychological behavior and notifies the NCC of any concerns and documents observations in the resident’s medical records. Check emergency equipment every week, i.e., O2, oral suctioning, ambu bags, CPR board to ensure good working order and report concerns to Maintenance and Nursing Administration. Assure that nursing unit is clean neat and functional (dirty utility room, clean utility room & kitchen). Ensure each resident is treated with dignity and respect and report any concerns with resident care and treatment to Administration in accordance with facility policy. Must follow the established infection control procedures. Participate in In-service education programs and in continuing education programs designed to keep abreast of changes in the nursing profession, keep LPN license current and provide a copy for personnel file. Job Summary The RN - Nurse Navigator acts as the primary point of contact and is responsible for monitoring and coordinating care for cancer patients and families. Job Summary The RN - MDS Coordinator will play a crucial role in ensuring accurate and timely completion of Minimum Data Set (MDS) assessments, collaborating with interdisciplinary teams to enhance resident care plans, and maintain compliance with regulatory requirements. Identifies resident acuity and needs, helping to determine specific care needs, and coordinates interdisciplinary care planning schedule and process. Core Job Responsibilities Coordinates completion of the MDS assessment process with the appropriate participation from other health care professionals according to CMS guidelines. Collaborates with the interdisciplinary team to develop individualized care plans based on MDS assessments. Schedules required MDS updates with care team, checks for completion and submits to CMS. Checks CMS reports to ensure submissions are complete. Creates care conference schedules and care plan schedules to ensure timely interdisciplinary care planning meetings, and to establish assessment reference period to assure accurate assessments. Tracks insurance dates. Schedules end of Medicare MDS when appropriate. Notifies billing, interdisciplinary care team and pharmacy of any changes. Monitors for any resident changes in condition that may change scores to increase reimbursement. Ensures completion of daily Medicare documentation and Medicare (re)certifications. Initiates and monitors appeals to Livanta. Prepares CMI (Case Mix Index) calculations and certifications for the State. Reviews and prepares QAPI reports from iQIES and CASPER. Evaluates care programs and initiates changes as necessary to ensure compliance with regulatory requirements. Communicates State and Federal regulatory revisions to facility management to maintain awareness/compliance. Performs audits, analyzes data and assists with plan of action to correct identified deficiencies. Reviews/analyzes QI/QM data for trends and indicators of negative outcome and initiates corrective action. Coordinates the processing of Medicare determination to assure timely and accurate billing of resident accounts. Consults with other departments as appropriate to collaborate in resident care, risk management and quality management activities. Provides education to staff on MDS/PRI and care planning. Education/Experience Requirements REQUIRED: Three (3) or more years of clinical experience in an acute or long term care care setting. Strong understanding of MDS processes, regulations, and reimbursement systems (RUGS and PDPM). Excellent communication and interpersonal skills. Ability to interpret financial and statistical reports. Highly proficient with MS Office, databases, medical software and the ability to learn new applications rapidly. PREFERRED: Three (3) years of experience working as a MDS/PRI assessor. Two (2) or more years of management experience in the health care field. Ability to read, write and speak in other languages.
