Protect patients, employees by adhering to infection control policies and protocols, medication administration and controlled substance regulations. Utilize universal precautions and practice at all times. Maintain a safe and clean working environment by complying with procedures rules, and regulations; calling for assistance from health care staff. Document patient care service by charting in patients’ medical records accurately and timely in accordance with licensure and regulatory/hospital standards. Maintain continuity of care with the nursing team by communicating plan of care and follow through with multidisciplinary team ensuring that patient administration of care for all services provided are organized, planned and delivered. Maintain patient confidence and protect operations by keeping information confidential. Maintain nursing supplies inventory checking stock to determine inventory and PAR level; anticipating needs supplies; placing and expediting orders for supplies, verifying receipt of supplies; using equipment and supplies; using equipment to accomplish job. Maintain professional and technical knowledge by attending educational workshops, mandatory in-services; reviewing professional publications; establishing personal networks; participating in professional societies. The nurse participates in the advancement of the profession through contributions to practice, education, administration, and knowledge development. Engage in quality improvement initiatives including continuing education, participation in hospital-wide and unit specific teams, identifying nursing practice areas for quality improvement and safe outcomes. Maintain a cooperative relationship among the healthcare team by communicating information; responding to request; building rapport; participating in team problem solving. What you'll do The primary purpose of this position is to establish, organize, direct, and implement the Cardiac Rehabilitation Program in accordance with Federal, State, and local guidelines. The Cardiac Rehabilitation Nurse evaluates & treats patients with a wide variety of cardiac disabilities, in an outpatient setting, helping to achieve risk factor reduction in cardiovascular disease using various therapeutic exercises, activities, and educational classes. The Cardiac Rehab Nurse is responsible for documenting all patient care appropriately in the medical record to comply with the policies and procedures of the department and JCAHO requirements. Responsible for orientation and initial assessment of participants in the cardiac rehabilitation program Develops plans of care for new participants Documents initial assessment and daily participant information and progress Leads and/or delegates participant education sessions based on staff expertise and identified participant needs Develop and maintain current educational materials for patient education Assess participants pre-, during, and post- exercise Supervises exercise sessions and/or delegates based on staff experience Obtains monthly required approval of individualized treatment plans from program medical director, per Medicare guidelines Monitors participants for inappropriate or abnormal responses to exercise Makes appropriate referrals and /or recommendations to participants, the participant’s personal physician, and/or program Medical Director as needed or as requested by patients Reports any significant participant conditions (dysrhythmias, abnormal blood pressure response, change in participant’s status, etc.) immediately to the physician supervising the program Initiates ACLS measures and/or emergency care according to facility policy Checks AED equipment and other emergency supplies and equipment for proper function and adequate supply per facility policy Set up and supervise emergency drills and mock codes on a routine basis giving appropriate feedback and constructive criticism where appropriate Optimize reimbursement for cardiac rehabilitation services through effective communication with all cardiac rehabilitation staff, including support and/or office staff Coordinates and/or maintains Continuous Quality Improvement program and participates in or delegates participation in quality reporting program per facility policy Maintain current knowledge in the field of cardiac rehabilitation through resources including but not limited to: cardiac rehabilitation literature, in-services, continuing education courses and/or conferences Responsible for coordination of any routinely occurring meetings, including: organizing and planning guest speakers, education, food, and/or public communications What we need from you Education: Must be a graduate of an accredited school of nursing, required *** All RN’s hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire.
