Applies warm and cold compresses, consistent with facility procedure. Performs irrigations, consistent with facility procedure. Performs other procedures related to skin care, as necessary. Administers medications correctly and safely; correctly identifies medication in terms of action, dosage, side effects, and implications for the patient. Meets standard on medication administration examination. Demonstrates preparation of local solutions. Administers and documents medication correctly. Educates patients and significant others related to drug and food interactions. Participates in patient and family education. Provides patient with an explanation of his/her condition as indicated. LPNs in the Outpatient Hemodialysis Setting An LPN can practice in the chronic hemodialysis outpatient department under the direct supervision of an RN. The LPN will participate in educational programs on a regular basis, including initial orientation and continuing in service training to care for patients with central venous access devices (CVAD). The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment.
