Accountabilities JOB DESCRIPTION Budget Responsibility: Assists management with maintaining par inventory levels in a fiscally responsible manner. Authority/Decision Making: Takes appropriate clinical actions using available resources to achieve positive outcomes for patients, visitors and staff. Independently prioritizes daily tasks and assignments. Identifies and recommends opportunities for improvement in order to ensure patient safety and quality outcomes. The Registered Nurse (RN) in the ambulatory practices functions as an integral member of the interdisciplinary healthcare team in providing patient care. The RN systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care. The RN utilizes evidence to drive clinical decisions and practice related to individual and population health. The RN considers factors related to safety and effectiveness in planning and delivering care. The RN provides care in accordance with the NJ Nurse Practice Act. The RN must successfully pass all required training to obtain security access to perform role, including EMR training, AMG clinical training, department specific clinical competencies, among others. Will have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families. Must have excellent communication, organization, and computer skills. Must have the ability to work in a highly specialized area with a focus on patient safety and efficiency. Job Description The Hospice Liaison/Admissions Nurse is responsible for coordinating hospice admissions from hospitals, healthcare facilities, physicians, and community referral sources. Working closely with patients, families, physicians, discharge planners, and hospice leadership, this role facilitates a smooth transition to hospice services while ensuring compliance with clinical, regulatory, and reimbursement requirements. Current and active New Jersey RN licensure is required. Current BLS, ACLS, and PALS, APLS, or ENPC courses are required. BLS certification (American Heart Association) is required upon hire. PALS/APLS/ENPC upon hire or within 1 year of the start date. Must be I.V. certified from an approved facility or complete the requirements for I.V. certification within 90 days of hire to the unit. Completion of all Orientation requirements of Cooperman Barnabas Medical Center. Successful completion of all Orientation Programs. Certifications And Licenses Required The American Heart Association Basic Life Support (BLS) Certification Active New Jersey Registered Nurse License Scheduling Requirements Day Shift, 12.5-hour shift, 6A-7P 37.5 hours weekly Required to work every other weekend and holiday rotations Essential Functions The Registered Nurse provides nursing care in the health care system within the scope of practice defined by the Nurse Practice Act and regulatory standards and assures awareness of ethical standards of practice as defined by the ANA code of ethics. The Registered Nurse assures safety in practice and is inclusive of the principles of evidence-based practice by maintaining a realization of their own individual level of knowledge and experience and assumes responsibility for advancing knowledge of self and others in the patient care team by communicating with and utilizing resources and programs available. The Registered Nurse is responsible for reporting identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors. The Registered Nurse is responsible for knowledge of the patient population served. The Registered Nurse is responsible for supervising and directing the delegation of tasks to appropriate team members and collaborates with other patient care providers. The Registered Nurse is responsible for consistently setting and evaluating priorities through appropriate interventions with measurable outcomes toward improvement. The Registered Nurse participates in patient teaching and discharge planning. Participates in unit Performance Improvement evaluation activities. The Registered Nurse is an individual practitioner and an interdisciplinary team manager, who utilizes the nursing process to integrate all aspects of the provision of care and individualizes the plan of care to address a specific patient’s needs.
