Position Overview: This role is based at a healthcare facility in the Bronx, supporting admissions, care planning, and overall nursing department operations while ensuring high-quality resident care. Admissions experience is a must! Duties & Responsibilities: Conduct admission assessments for new residents and hospital returns; initiate care plans. Review PRI documentation and coordinate all aspects of the admission process with the interdisciplinary team. Supervise or direct resident care as assigned. Requirements/Qualifications: Active, unrestricted New York State Registered Nurse (RN) license. Long-term care (LTC) experience required; admissions experience needed. Strong clinical assessment and critical thinking skills. Ability to make sound, independent decisions.
