At Houston Methodist, the Registered Nurse (RN) II position is a licensed staff nurse, an experienced clinician, functions at the Competent to Proficient stage of Benner's model of clinical practice. When hospital patients transfer to hospice, they may stay in the hospital, but care will transfer to the AccentCare Hospice team. Serve as the primary clinical resource for hospice education, eligibility, and appropriateness. Conduct patient assessments and coordinate the admission process. Partner with Admissions, Clinical, and Care Teams for smooth transitions. Advocate for patients and families during goals‑of‑care conversations.
