JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Fill out complete accident/incident reports. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Obtain /assist in wound specimen collection as ordered by the physician. Update the family/responsible party and attending physician for any changes on wound assessment. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer and be able to evaluate the delivery of care. Attends and actively participate in customer care plan conference. Consult with other nurses, management, and other related health professionals to assist in assessing, planning, and delivering and evaluating patient care. Plan, conduct and participate in In-service Education Programs, workshops, seminars, conferences, and courses. Make written and oral reports including recommendations concerning the wound care program. Ensure that the wound assessment reflects that the care plan is being followed. Participate in the development, maintenance, and implementation of the facility’s Quality Assurance program for the Wound Care Program. Coordinate facility wound audits with the nursing staff. Assist to update activity boards through direction of the Activity Director, with a monthly theme that is creative, neat, and clean. Facilitate community involvement, such as inviting the community to events and programs. Assist in resident outings, which can include but not limited to shopping centers, movies, restaurants, community events, etc. that may also entail driving residents to and from the resident outing. Follow Activity staff schedules with a team approach and continuity of care. Work as team with staff and volunteers. Must maintain structure of activity schedule and conduct all activities on-time; May not cancel an activity without permission of Activity Director. Must maintain equipment and supplies in good clean order. Must notify Activity Director within 48 hours if any new materials are needed for scheduled activities and when repairs are needed. Overview: Job Summary: Provides personal care services under the direction of the Registered Nurse or Therapist. The Home Health Aide is assigned to specific clients by the Registered Nurse or other appropriate professional and performs services for clients as necessary to maintain their personal comfort Responsibilities: Job Duties: Performs simple procedures as an extension of therapy services under the direction and supervision of the Range of motion exercises Assistance in ambulation or exercises Performs personal care activities, including but not limited to: Bathing Shampooing Skin care/nail care Oral hygiene Shaving Dressing Performs household services essential to health care at home, including but not limited to: Meal preparation/feeding Laundry Light housekeeping Assists in the administration of medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse (per state nurse practice laws and Agency policy).
