Evaluates care given and adjusts plan of care to meet the changing needs of the patient. Documents appropriately in patient care records. Prepares and administers medication/treatment according to the physician's orders, utilizing professional knowledge base to assure patient safety. Works collaboratively with other patient care and multidisciplinary team members to provide patient centered care. Documents or ensures documentation of all of patient care from admission to discharge according to hospital policies. Incorporates discharge planning into clinical pathway or plan of care to provide patients with smooth transition from hospital to home. Provides cost-effective care by appropriate utilization of time, supplies and personnel. Evaluates patient care interventions and collaborates with all team members to make changes as necessary. Initiates and/or participates in patient care conferences. Assumes the responsibility to interact on a consistent basis with the patient and family providing coordination of the care plan for those patients whose needs do not require the expertise of a Case Manager.
