Job Duties: Conduct s complete nursing assessment of all admissions including risk assessments (SAO, elopement, assault, suicide ); educational needs, and medication review. Complete s all required nursing documents within established time frame . Actively participate s in treatment team meetings & decisions, collect s & include s data in care plan updates, asses ses patient needs, design s and implement s programs, and document s / evaluate s patient responses . Document s nursing care and treatment progression through timely & accurate chart entries & nursing treatment plan updates . Complete s treatment plans as per policy . Appropriately notify Physician of changes in condition and other patient issues, as per policy . Assess es , plan s , implement s , and evaluate s patient care and the milieu. Appropriately monitor s patients through detox, including CIWA and COWS assessments. Provide s oversight of completion of physician orders, including labs, EKG’s and x-rays . Ensure s that medication & treatment are administered as prescribed and that documentation reflects the care provided . Complete s medication consents from patient or guardian , as per policy . Provide s leadership, supervision, and direction to Mental Health Technicians and LPNs. Provide s oversight of q15 observations as evidenced by signatures on q15 observation rounding’s . Conduct s rounds. Provide s and direct s milieu management. Actively assist nurses and MHT’s, keeping safety foremost . May need to assist patients with personal hygiene and ADL ’ s as needed . May provide q15 observation round coverage at times. Assess es , direct s and intervene s in crisis situations . Recognize s escalation and take s preventive steps to reduce incidence of restraint and seclusion. Provide s and document s patient and family education . Leads nurse education groups and completes documentation . Initiate s and account s for Special Treatment Procedures . Provides a thorough and accurate report to the oncoming shift and completes an appropriate hand-off of the patients and q15 observation forms . We are seeking a full-time, benefited Field Nurse Case Manager to cover Panama City, FL and surrounding communities such as Marianna, Chipley, and Dothan, AL. Schedule & Salary: This position will begin at 30 hours per week , with the potential to increase to 40 hours . The salary range for 30 hours/week is $68500 - $74000 annually . A Field Case Manager role is a home-based position with travel, up to 2 hours one-way (four hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.
