Join Proactive MD as a Family Nurse Practitioner Proactive MD is seeking an experienced Family Nurse Practitioner (FNP) to provide comprehensive primary care at our employer-sponsored health center in New York, NY. Position hours: Monday--Friday 8:00am - 5:30pm, no weekends. (40hrs/week). What You'll Do As a Family Nurse Practitioner, you will: Provide comprehensive primary care to adult and pediatric (2+) patients in an employer-sponsored health center Perform preventive, acute, and chronic care visits Conduct physical examinations, health assessments, and comprehensive medical histories Diagnose and treat common acute and chronic medical conditions Develop individualized treatment plans in collaboration with patients Provide health education, preventive care, and wellness counseling Order and interpret appropriate laboratory and diagnostic testing Prescribe medications in accordance with applicable state and federal regulations Coordinate referrals to specialists and other healthcare resources when appropriate Develop strong, long-term relationships with patients Document patient encounters accurately in the electronic medical record Maintain compliance with HIPAA, OSHA, and clinical quality standards Collaborate with the onsite clinical team to provide coordinated, high-quality care Support clinic workflows and contribute to an efficient, team-oriented environment Champion the Proactive MD Mission, Vision, Values, and Patient Promise in every patient interaction Qualifications Required Master's degree in Nursing (MSN) Family Nurse Practitioner (FNP) certification Minimum of 3 years of experience in family practice or primary care Current Registered Nurse (RN) license Current Nurse Practitioner license in the state of practice Current Basic Life Support (BLS) certification through the American Heart Association (AHA) or American Red Cross (ARC) Appropriate prescriptive authority under applicable state and federal laws Experience using electronic medical records (EMR); Proactive MD uses athenahealth Strong computer skills, including experience with internet, spreadsheet, and word-processing software Strong clinical judgment, problem-solving, and workflow-management skills Knowledge of HIPAA, OSHA, and clinical quality standards Ability to work effectively with patients and colleagues from diverse backgrounds Preferred Experience providing comprehensive primary care in an employer-sponsored, occupational health, or workplace health setting Experience managing both acute and chronic conditions in an outpatient primary care environment Interest in preventive medicine, population health, and patient engagement Why Join Proactive MD? SCHEDULE: 8:00am-4:30pm EDUCATION: Graduate from an accredited School of Nursing with a Bachelors Degree in Nursing or graduate from an accredited School of Nursing with a Bachelors Degree in a healthcare related field. Evidence of continuing professional education. EXPERIENCE: Five years experience as a medical/surgical nurse, a portion of which should be in the outpatient setting. Experience in occupational health preferred. Computer skills: Proficient in Microsoft Office software: Word, Excel, and Email. SPECIAL SKILLS: Physical and cognitive requirements: Position’s physical requirements necessitate a full range of body motion including but not limited to, physically assisting patients, requiring stretching, bending, lifting, sitting, kneeling, stooping, standing, walking, pushing and pulling. Position is classified according to the USDOL as Very Heavy Work which is defined as exerting in excess of 100 pounds of force occasionally, and/or in excess of 50 pounds of force frequently, and/or in excess of 20 pounds of force constantly to move objects. The National Institute of Safety and Health (NIOSH0 guidelines, which recommends that the average worker not lift more than 51 pounds without assistance, will be followed. Manual and finger dexterity and hand-eye coordination are also required as is constant talking and listening in English. Must demonstrate effective oral and written communication skills, including speech clarity, understanding the information relayed, deductive and inductive reasoning skills (the ability to apply general rules to specific problems to produce answers that make sense and the ability to combine pieces of information to/from general rules or conclusion respectively). Considerable walking, standing, sitting, reaching, filing, typing and photocopying required. Ability to perform work accurately and pay attention to details, often changing from one task to another without loss of efficiency or composure. Ability to function competently in stressful situations and changing work environment related to changing needs of patients with acute, chronic and complex and/or terminal disease processes. Ability to work cooperatively within health system, multidisciplinary team members (i.e. MD’s, Rehab, Respiratory, DI, Laboratory, etc). Ability to utilize effective time management skills to set priorities to perform responsibilities, problem solve and deliver care. Current Occupational Health Nurse Certification or OHNC Certification within 2 years of hire into the position. CPR (BCLS) Certification with AED Certification, Venipuncture. Strong knowledge of the principles, practices, rules, requirements of Infection Control, Health & Wellness, Case Management, Medical Surveillance. Position Summary Performs direct clinical activities in a courteous, caring and respectful manner. These include patient intake, blood draw, pharmacy calls, EKG and any tests required by the provider. Responsible for accurately documenting an EMR system in accordance with licensure and Valley Medical Group (VMG) policies and procedures. Maintains close working relationships with all practice staff to ensure proper workflow, care and treatment. Performs a variety of front desk activities that support the Practice and professional staff to include patient registration, billing, insurance, and safe visit protocols as directed or requested by Practice Manager/Supervisor. Education High school diploma or equivalent and completion of a formal Medical Assistant program recognized by the State of New Jersey to include completion of formal coursework in office management, database management or business.
