Overview Manages, coordinates, facilitates and supports the members of the interdisciplinary team in provision of hospice care to patient/families of VNS Health Hospice Care Program, including After Hours. Ensures the implementation of clinical/quality improvement initiatives for hospice services in compliance with regulatory requirements. Demonstrates knowledge and commitment to excellence in clinical practice and customer service. Works under general direction. *This role includes a $15K sign-on bonus, payable in two installments throughout the first year of work with VNS Health. Ensures that support and direction to patient’s families are provided in accordance with the patient’s Interdisciplinary Plan of Care/Hospice Care mission, objectives, policies and procedures. Participates in Hospice Team meetings, IDG management meetings, and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. Ensures full regulatory compliance with Hospice Conditions of Participation and standards through review, monitoring and audit (ad hoc / regular) of files, processes and procedures. Collaborates with Quality Improvement/Education in the development and implementation of quality improvement and educational activities to ensure professional practice standards are consistently met for each member of the team. Participates in the design, implementation, evaluation and modification of quality initiatives and educational plans. Initiates, coordinates, performs and assists in the analysis of clinical, regulatory and fiscal practice. Assists leadership in achieving Hospice Care goals, objectives, and fiscal targets by providing support with compliance and/or innovation to achieve improvement. Manages the day to day operations of the department, which includes effective caseload or visit management, achievement of productivity and revenue/expenditure targets for Hospice Care. Completes all work assignments, which includes audits, reports and projects. Performs all duties inherent in a managerial role. Ensures effective staff training, evaluates staff performance, provides input for the development of the department budget, and hires, promotes, and terminates staff and recommends salary actions as appropriate. Facilitates and manages the interdisciplinary group for provision of an effective and comprehensive hospice services to patient/families of VNSNY Hospice Program. Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: License and current registration to practice as a Registered Professional Nurse in New York State required Licensed and currently registered to practice as Nurse Practitioner in New York State and DEA license preferred Education: Bachelor's Degree in Nursing from an approved program accredited by the National League of Nursing or Health or Human Services field or the equivalent work experience required Work Experience: Minimum of three years clinical experience in hospice required Effective oral, written and interpersonal communication skills required Clinical management experience preferred Knowledge of personal computer operations, including MS Office applications preferred Pay Range USD $98,200.00 - USD $130,800.00 /Yr. Location Brooklyn, New York Shift: Day (United States of America) Description: Nurse Practitioners: Setting Standards, Changing Lives NewYork-Presbyterian Medical Groups (NYPMG) are part of the physician enterprise of NewYork-Presbyterian, one of the nation’s most comprehensive academic health care delivery systems affiliated with two world-renowned medical schools. Through primary care and multispecialty medical practices, the physicians and other health care professionals of NYPMG, in collaboration with Columbia Doctors and Weill Cornell Physicians, provide families with high quality, patient-centered care close to home in the New York metropolitan area. This position is approximately 50% clinical patient care and 50% nuclear stress lab responsibilities. Nurse Practitioner - Cardiology - Brooklyn Medical Group Clinical Patient Care : Provide comprehensive outpatient cardiology care in collaboration with a board-certified cardiologists. Perform history and physical exams, manage chronic cardiovascular conditions (e.g., hypertension, heart failure, coronary artery disease, atrial fibrillation), order and interpret diagnostic tests, and develop treatment plans. Adjust medications, provide patient education, and coordinate follow-up care. Document thoroughly in the electronic medical record. Nuclear Stress Lab Duties : Prepare patients for nuclear stress testing (exercise and pharmacologic). Explain procedures clearly and compassionately to patients, addressing questions and concerns. Administer and monitor pharmacologic stress agents (e.g., regadenoson, Adenosine) . Supervise and assist with patient monitoring during stress testing, including IV placement, vital signs, and ECG monitoring. Work closely with the attending cardiologist, nuclear medicine technologists, and nursing staff to ensure safe and efficient test completion. Assist with preliminary reporting and quality assurance as needed. This is a full time position located in Bay Ridge Brooklyn. The schedule is Mon/Wed/Fri 9:00AM-5:00PM and Tues/Thurs 10:00AM – 7:00PM. One Saturday a month required. Preferred Criteria Prior experience working in an outpatient cardiology practice or stress lab. Comfort with IV starts and cardiac rhythm interpretation. Strong communication skills and ability to educate patients effectively. Their frequent contact with patients assists the nurses in utilizing the nursing process. Qualifications: 1. Skilled Nursing Facility (SNF): Requires successful completion of Nurse Aide Training Program. Maintenance of certification is required. 2.
