Licensed Practical Nurse (LPN) Duties and Responsibilities: Provides licensed care to assigned residents as ordered by physician and in accordance with facility, federal, state and local standards, guidelines and regulations Provides and receives nursing reports upon reporting in and ending shift duty hours. Ensures assigned work area (i.e. nurse station, medication cart, medication room, etc.), resident care rooms and treatment areas are maintained in a clean and sanitary manner. Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations. Provides required resident assessments/observations and interventions, reviewing care plans to ensure appropriate care. Takes and records residents blood pressure and vital signs, administers catheter care, administers IV therapy to residents as ordered within license limitations, and provides respiratory care, e.g. oxygen, nebulizer treatments. Receives and transcribes physician’s orders to resident’s charts, Kardex/Care Medication Administration Record (“MAR”), treatment/care plans, as required. Prepares and administers medications and performs treatments as ordered by the physician. Charts nurse’s notes in an informative and descriptive manner that reflects the care provided as well as the resident’s response to the care. Completes the required forms upon admission, transfer, and/or discharge. Ensures the resident’s environment is as free from accidents and hazards. Immediately corrects/addresses potential safety risks as prudent and warranted. Makes written and oral reports/recommendations concerning the activities of shift as required. Ensures that all aspects of resident care plans are implemented and maintained. Make recommendations for care plan changes to the IDT per professional standards. All other duties, as assigned. Licensed Practical Nurse (LPN) Skills and Abilities: Excellent written and verbal communication skills in English. Ability to multi-task and remain calm under stress. Analytical and problem-solving skills. Understands and follows infection control standards, policies and procedures. Must be able to adapt to a flexible work schedule and occasional overnight travel. Demonstrate basic computer knowledge and ability with an aptitude to learn company applications. Licensed Practical Nurse (LPN) Education and Experience: Current CPR Certification required. Possess a current license to practice in the State as a Licensed Practical Nurse. 1 year of experience preferred. Long-Term care experience preferred. Physical Requirements: Walking, reaching, bending, lifting, extended sitting, grasping, fine hand coordination, pushing and pulling, all with or without the aid of mechanical devices is required. SUMMARY: The Nurse Manager performs professional supervisory nursing practice in the care of the physically and mentally ill. Work involves responsibility for supervising the provision of quality nursing care given at the facility. The position is responsible for maintaining the quality and adequacy of nursing practice; for coordinating nursing programs; for recognizing and assisting in solving professional, administrative and supervisory problems. Position exercises considerable judgment in applying professional knowledge in solving nursing problems within established policies and practices. ESSENTIAL DUTIES AND RESPONSIBILITIES : · Determines the optimal allocation of present staff, per shift and assignment on each unit is present. · Coordinates the care of patients on assigned shift. Communicates shifts concerns by unit to appropriate staff (via e-mails, voice mail, etc.). · Review of 24 Hour Report and review patients as indicated for change of condition. · Serves as administrative authority for the smooth function of all units. Notifies Director or Nursing, Assistant Director of Nursing, Scheduler/Central Supply of urgent concerns during the shift. When the concern is non-urgent, notifies manager by phone message or e-mail. · Receives employee, resident and family grievances. Description Weekend-RN-Medical Surgical Full-Time Days The Staff Nurse Inpatient reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards. Responsibilities: A clinically competent, registered, professional nurse who prescribes, delegates and coordinates nursing care to the acutely ill patient through the nursing process of assessment, planning, intervention, implementation and evaluation. Effectively interacts with patient, significant others, and other health team members, while maintaining ANA standards of professional nursing practice. Collects and analyzes data, identifies expected outcomes, develops and implements an individualized plan, coordinates care delivery and evaluates progress toward attainment of outcomes. Directs care and determines the appropriate utilization of any assistant involved in providing direct patient care. Collaborates with patient, family, physicians and ancillary staff. Creates a documented plan, focused on outcomes and decisions related to the care and delivery of services that indicates communications with patients, families, and others. Participates in shared governance, committee work, and unit projects. Evaluates and documents the patient’s responses to nursing interventions in the health record and properly individualizes the plan of care based on the evaluation of findings. Utilizes the best available evidence, including research findings to guide practice decisions.
