This is a full-time, primarily outpatient position supporting a high-volume Gastroenterology physician and helping expand access to high-quality specialty care. The NP will work closely with Dr. Christine Yu and the multidisciplinary practice team to provide comprehensive patient care, clinical triage, patient communication, and provider support. This role offers an opportunity to increase patient access, improve continuity of care, and allow the physician to focus on complex cases and procedures. The position is primarily outpatient, with the potential for inpatient responsibilities and hospital rounding in the future based on practice needs. Key Responsibilities Provide comprehensive clinical care to Gastroenterology patients in the outpatient setting. Position Summary Support the referral, screening, and onboarding process for potential and newly admitted hospice patients. This role assists in reviewing incoming referrals, gathering essential clinical information, and coordinating with physicians, referral sources, patients, and families to facilitate timely hospice admissions. Provides clear explanations of hospice philosophy, services, and the admission process to ensure understanding and promote a smooth and supportive transition to hospice care. Working collaboratively with the interdisciplinary team, this role helps ensure that patients who are appropriate for hospice services are identified, informed, and efficiently connected to care. Education Graduate of an accredited school of practical nursing. Experience Completion of the Agency’s Orientation Program of the Valley Home Care. Skills Current and valid NJ license and CPR certification. Ability to use critical thinking skills to set priorities, effectively problem-solve, and perform work accurately with attention to details, often changing from one task to another w/o loss of efficiency or composure.
