DEPARTMENT DESCRIPTION The Hospice teams at Samaritan Health Services support patients and family members through the final stages of their illness. Evergreen Hospice serves patients in Linn, Benton, Marion, Polk, Lincoln and Tillamook Counties. These services are offered in the home setting which may include the patient’s own residence, skilled nursing facilities, assisted living facilities and adult foster homes. Services offered include intermittent skilled nursing visits (24-hour on-call nursing services available), pain and symptom management, medical social services, counseling, spiritual support, hospice aides, trained hospice volunteers and bereavement support. Medications and equipment are included as appropriate. EXPERIENCE/EDUCATION/QUALIFICATIONS Current unencumbered Oregon RN Licensure required. Must be able to pass Criminal Records Check upon hire and every three (3) years thereafter. Healthcare Provider Level BLS required. BSN and/or previous HH/Hospice or case management experience preferred. Previous broad-based nursing experience to ensure specialized skills needed for HH/Hospice Nursing preferred. AGH Hospice Outpatient: This position requires the use of the employee's personal automobile. If you’re looking for part-time work just one day a week, this position is ideal. In this role, you’ll work one day per week between 7:30 a.m.–3:30 p.m. (day of the week is flexible - MONDAY is preferred). We’re a small clinic with a big impact—providing Primary Care, Acute Visits, and light Occupational Health services . What You’ll Do Assesses patient care needs, conducts nursing evaluations, and provides treatment within scope of practice Collaborates with physicians and other healthcare practitioners for further medical evaluation and care as needed Performs and documents medical tests, screenings, and treatments as defined in site contract (EKG, audiometry, spirometry, vision) Administers medications and emergency care within legal scope of practice Maintains medical equipment, ensures a clean and safe environment, and follows infection control procedures Maintains patient health records to ensure accurate and up-to-date records Educates patients on health promotion and participates in follow-up care Performs other duties as assigned What You’ll Bring Certificate from an accredited Nursing School or associate degree (A.S.) is required; Bachelor's degree (BSN) from four-year college or university is preferred Currently licensed Registered Nurse in state of practice required Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services Certification in Occupational Health Nursing (COHN/COHN-S), Emergency Nursing, or Case Management preferred and may be required for some sites At least three years of experience as a Registered Nurse in an emergency room, occupational health clinic or other ambulatory care clinic Work-life balance is at the foundation of how decisions are made and where Premise is headed.
