Position Objective: Contributes to the provision of high-quality, cost-effective healthcare as a provider of direct and indirect patient care and by effective of the health care team. Nurse-Patient Family Relationships • Demonstrates the ability to assess the patients/family’s learning needs, readiness to learn, learning style, and presence of barriers to learning. • Demonstrates the ability to develop, implement and evaluate teaching plans for patient populations in unit specialty in accordance with applicable standards. • Demonstrates the ability to apply knowledge of growth and development across the life span to the care of patients. • Provides direct patient care to patients and families in a culturally, developmentally and ethically appropriate manner. • Plans of care address the physical, psychosocial, spiritual and learning needs of the patient/family. 3. Clinical Scholarship • Participates in QI, CPI and risk management activities at the unit, department or organizational level. • Demonstrates the ability to effectively perform and improve all processes to achieve excellence with regard to Luminis Healths quality standards and benchmarks. • Supports the use of evidence-based guidelines and organizational policies and procedures to promote safe patient care and a safe practice environment. 4. Clinical Leadership • Participates in unit shared governance according to departmental standards. • Participates in the education and orientation of new staff. • Delegates patient care activities as appropriate; evaluates delegated activities for expected patient care outcomes. • Employs real time computer documentation when completing patient records. Educational/Experience Requirements: • Graduate of an accredited school of nursing BSN required, MSN preferred • Adherence to the credentialing requirements of Luminis Health as stated in the nursing bylaws. • Minimum 2-3 years of experience in critical care, emergency room required, 3 or more preferred • Cath Lab/Interventional radiology-specific experience preferred • Experience with conscious sedation and cardiovascular medications – highly desirable Required License/Certifications : • Current licensure as a registered nurse by the Maryland Board of Nursing • BLS, ACLS- American Heart Association Health Provider • If serving as a bilingual/multilingual interpreter, certification in Medical Interpretation is required to perform the duties of a qualified interpreter. • Certification preferred Working Conditions, Equipment, Physical Demands: There is a reasonable expectation that employees in this position will be exposed to blood-borne pathogens. The CICU staff includes an integrated cardiac team with all sub-specialists readily available and accessible. Children’s CICU is home to an Extracorporeal Life Support Organization (ELSO) center of excellence for ECMO services in children. Job Summary: The Patient Care Coordinator is responsible for overseeing the management and coordination of care for the acute inpatient population. The PCC collaborates with rounding MAPMG Hospital Based Service Physicians, patient/family, nursing, utlization review and other members of the healthcare team to assure continuum of patient care progression for clinical and cost-effective outcomes. The PCC facilitates and coordinates with community providers and ambulatory case managers to assist with the appropriate level and transition of care for a safe discharge and preventing a re-admission. Essential Responsibilities: Completes an initial face-to-face assessment for every admitted member to identify discharge needs within 24hrs of admission. Document in KPHC and communicate the assessment outcomes to determine the appropriate transition plan with MAPMG physician healthcare team and patient/family. Active participant in daily Care Without Delay (CWD) rounds reporting on patient progression towards the established discharge plan. Review and document discharge plan in accordance with KP discharge planning documentation policies, facility policies, and regulatory requirements. Document any updates, care progression and barriers to discharge daily, and as indicated on assigned patients. Manage timeliness of care progression with physician and nursing staff to prevent avoidable delays and or days. Collaborate with Social Worker to coordinate, long-term care, assisted living, financial assistance, and other services, as required. Send referrals/communicate with in-network vendors for coordination of post-acute levels of care such as Home Health, DME, IV infusion, SNF, Sub-Acute and Acute Rehab. Timely identification, recording, and escalation of delays in care and barriers to discharge. Provide solutions to correct delays and recognize systemic patterns that require corrective action. Assure follow up appointments and referrals to ambulatory case manager for high-risk patient population are scheduled and communicated to patient/family prior to discharge. Observe all facility safety policies and procedures (infection control, Members Rights policies, and any regulatory requirements) Participate in Quality Assurance duties and implementation of programs to improve care Quality Indicators.
