The Certified Nursing Assistant (CNA) helps patients/residents with comprehensive and personalized health care needs under the supervision of a nurse. The CNA assists patients/residents with activities of daily living to include bathing, dressing, eating, grooming, toileting and exercising. Certified Nursing Assistant (CNA) Duties and Responsibilities: Under the direction of the Director of Nursing or CNA Supervisor, the CNAs work closely with the nursing staff to ensure coordination of patient care. Provides physical support for patients/resident’s daily activities and personal hygiene, including bathing, dressing, grooming, transferring to/from bed, toileting, walking, standing, or exercising. Assists patients with daily medication and supplements. Ensures proper resident/patient diet based on their dietary restrictions. Examines patients for open wounds, bruises, or blood in urine. Notes and communicates patient/resident behavior and changes in mood. Cleans rooms and bed linens. Checks and records vital signs, including blood pressure, pulse, temperature, and respiration rate under the supervision of a licensed nurse. Answers to patient calls and needs. Familiarity with symptoms and signs of improvement for a number of medical conditions. Ability to read, write, speak and understand English. We are seeking an experienced Nurse Case Manager interested in working with injured workers and worker’s compensation cases to cover the Chicago, IL, greater area, including northern Indiana. This role is a home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Qualifications Current, unencumbered registered nurse (RN) license in IL Willingness to obtain IN RN license Experience in worker’s compensation catastrophic case management is preferred. Due to the complex nature of pain, RN will evaluate and treat pain in a prompt, caring fashion and reassess after each intervention, charting per UCMC protocol. RN maintains hospital required nursing & ICU unit specific competencies needed to provide proficient nursing care; RN will provide continuity of care for infant, pediatric or adult patients based on area of specialty. This includes assisting in the care of critically ill patients during sedation/procedural encounters. Administering high-risk infusion and transfusion (i.e. chemotherapy medicines).
