The Christ Hospital Home Health Care Schedule: Mon-Fri (8:00 AM - 5:00 PM) SUMMARY The Licensed Practical Nurse (LPN) - Territory provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM within specified geographical territory. The LPN educates patients and their family members and ensures the safety of the patient. The LPN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations. The LPN is responsible for all practices and duties within the scope of practice as outlined by the state. KEY RESPONSIBILITIES Provides skilled nursing care in a patient’s home as ordered by the attending physician. Performs care by following order sets and goals made by RN. Observes and monitors patient conditions. Notifies RNCM of any changes in patient condition. Administers medication as prescribed by the physician. Counsels patient and family on the disease/injury processes and how to manage. Incorporates patient and family in development of plan of care. Helps decrease re-hospitalizations by teaching the patient on preventative measures and making good decisions. Listens to patient and family members. Completes all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines. Documents all aspects of treatment, assessments, and patient education. Maintains active LPN license in state of employment. QUALIFICATIONS Minimum of one year of experience as a LPN in an acute care setting is required; two years is preferred. Home care experience is desired. Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers. Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients. Knowledgeable on nursing best practices. Ability to make appropriate nursing judgments. Ability to identify a situation and handle it with the best possible solution. Detail-oriented and observant. Job Description UC Health is hiring a Full Time Registered Nurse (RN) for the Surgical Oncology unit for the Night Shift at the UC Medical Center. Be a part of a growing Surgical Oncology program, by making an indelible impact cancer patient’s lives through caring for them with the most advanced treatment. The Surgical Oncology nurse will support the Medical Center’s 24-bed Surgical Oncology unit and Level I Trauma Center, delivering high-quality care to patients based on assessed needs, established standards of care and according to policy and procedures. UCMC is at the forefront in treating patients with pancreatic and liver diseases and is one of two centers that offer autologous islet cell transplantation for the treatment of chronic pancreatitis. 9CCP cares for these patients following ICU until discharge and when patients are readmitted with pain or infection issues. The unit also cares for acute care surgery, thoracic, gynecology/oncology, vascular and colorectal patients. Most of the patients at 9CCP are cared for under the direction of a surgeon. Most patients are admitted from PACU, ICU, ER or directly from their home. Nursing care is concentrated around recovery after surgery including mobility, monitoring intake and output and pain management. Job Description Join TriHealth — Where Neurocritical Care Meets Purpose Good Samaritan Hospital is a Comprehensive Stroke Center dedicated to caring for the most complex stroke patients. The 7Q Neurosurgical ICU at TriHealth Good Samaritan Hospital is a specialized 20-bed unit focused on providing intensive care for patients with complex neurological conditions. The unit cares for individuals with aneurysms, brain tumors, spinal injuries, abscesses, arteriovenous malformations, and traumatic brain injuries, as well as patients participating in various research protocols. The multidisciplinary team on 7Q is dedicated to delivering specialized care for these critically ill patients, using advanced monitoring techniques and individualized treatment plans. The orientation process for nurses joining 7Q includes comprehensive training to develop the specialized skills necessary for neurosurgical critical care. This includes EKG and ACLS training, as well as critical care classes, followed by a six-week competency-based orientation (CBO) preceptorship. Prior experience in neuro or critical care is preferred for those joining the 7Q ICU, and a minimum of one year of current medical/surgical or telemetry experience is recommended. However, new graduates may also be considered, particularly for the Neuro Stepdown unit, where they can gain foundational experience before transitioning to the ICU. Support leadership in Compliance efforts to meet and sustain the regulatory compliance efforts of the department and hospital. Be mindful and judicious in monitoring safety and departmental policies and procedures. Ensure all individual certifications and Safety College requirements are fulfilled in a timely basis. Work with department managers, nurse council and magnet representatives to identify and share safety best practices. Be mindful of CCHMCs processes and compliance with CMS standards and freely communicate safety and compliance concerns to leadership team members. Uses systems thinking - Sees the pieces or components; sees patient and family within the isolated environment of the unit; sees self as key resource to support the family through the healthcare experience through care coordination. Clinical inquiry & performance review to improve practice - Follows standards and guidelines; implements clinical changes and research-based practices developed by others; recognizes the need for further learning to improve patient care; recognizes routine or obvious changing patient situation (e.g., deterioration, crisis); in unusual care situations, seeks help when needed to identify patient problem. Uses self-appraisal performance review and peer review, including annual goal setting, for assurance of competence and professional development to promote safe and ethical practice using the nursing process. Demonstrates openness to coaching and mentoring from others. Facilitates learning by assisting with peer and student learning experiences. Works with department managers, nurse council and magnet representatives to identify, share, implement and evaluate safety best practices. Collaborates with team using shared decision making - Is receptive to the contributions of the interdisciplinary team in team meetings and discussion regarding patient issues. Participates in shared governance at the point-of-care as a member of unit-based councils or by demonstrating awareness and contributing to shared decision making (reading minutes, giving feedback and implementing council decisions). JOB QUALIFICATIONS Associate/Diploma prepared RNs from ACEN, CCNE or NLN CNEA accredited institution requires 2+ years of experience. If hired without a BSN or MSN, must enroll in a BSN or MSN program within 1 year of hire date/RN job date and complete program within 5 years of hire/RN job date. Preferred: BSN from ACEN, CCNE or NLN CNEA accredited institution or MSN. Active Ohio RN License. May be required to obtain other state licensure. Foreign graduates must have license validated through TruMeritTM per Ohio Board of Nursing code for Foreign Graduates. Job Summary The on-call LPN is responsible for coordinating care, providing direct care/support for hospice patients and their families in the home and various nursing facility settings. Flexible schedule, full time, part-time per diem. YOU DO NOT CARRY PHONES-triage nurse handles calls, you only go out if needed. Responsibilities and Duties On-call support after agency hours for hospice patients/families with regard to comfort, symptom management, medication management, DME needs, admissions, discharges, proper documentation and coordination of care between hospice interdisciplinary team & facilities. Qualifications and Skills Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing Self starter, excellent time management skills, ability to multi-task in a fast paced environment, dependable, autonomous worker, current clinical skills, flexible, professional and tactful. CPR certification required. Holds valid driver's license and current drivers insurance coverage. Schedule: Night shift On call Weekends
