JOB SUMMARY: The Clinical Assessor / Coordinator, LPN is responsible for reviewing sub-acute care and long-term care referrals received from surrounding areas hospitals/facilities. The Clinical Assessor / Coordinator, LPN identifies actual and potential mental and physical health problems and accurately determines the necessary classification and regimes to assist with the appropriate placement of a potential admission to the Gurwin Jewish Health Care System. HOURS: Monday through Friday 9:00am-5:00pm EDUCATION AND EXPERIENCE REQUIREMENTS Licensed to practice as LPN in New York State. Minimum of one year experience working in a hospital, long term care or certified home health agency (CHHA) DUTIES : Demonstrates knowledge and the capacity to accurately interpret the PRI/RUG Classification System. Properly manage referral case load and respond in a timely manner via Navi/Allscripts and eFax. Accurately assesses the classification and patient disposition upon screening. Ability to coordinate referral via EMR including verification of benefits Ability to coordinate the bed board/census when needed. Ability to assess potential patients at partnering hospitals.
