Description Medical Assistant II-Multispecialty-Brooklyn, NY-Full Time, NY-Offsite Provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Position Summary We are seeking a compassionate and experienced Admissions Registered Nurse (RN) to join our hospice team. The Admissions RN is responsible for evaluating patients for hospice eligibility, completing admission assessments, coordinating the start of care, and educating patients and families about hospice services. Determine hospice eligibility in collaboration with the hospice physician and interdisciplinary team. Complete all required admission documentation accurately and in a timely manner. Develop and initiate individualized plans of care. Educate patients and families on hospice philosophy, services, medications, and expectations. Coordinate admissions with referral sources, physicians, hospitals, nursing facilities, and case managers. Obtain informed consents and ensure all regulatory requirements are met. Collaborate with clinical staff to facilitate seamless transitions of care. Maintain compliance with Medicare, state, and agency regulations.
