Health Services Specialist I Full-Time Yuba City Health Center, Yuba City, CA General Duties: Provides direct service in all the health center specific areas including, sexual and reproductive health, abortion, as appropriate to scope of services and health centers; Provides patients with accurate information regarding Planned Parenthood services, including questions pertaining to contraception, options, and funding sources; Contributes to achieving health center productivity goals; Screens financial, medical and social history with correct documentation of pertinent information Demonstrates PPMM customer service standards; Solicits donations from patients per PPMM guidelines. Demonstrate PPMM workplace values and service standards. Follows all patient care standards Front Office Check-in & Check-out: Communicates professionally and accurately about services, payment and appointment scheduling. Provides paperwork to patients and secures payment and insurance information as needed for billing purposes. Enters accurate patient information into Electronic Practice Management System (EPM) Makes Health Center appointments, provides information and appropriate referrals Contacts insurance companies to obtain benefit information and authorization for services Ensures accuracy of charges and posts charges to EPM for all pay types following established practices and procedures. We are seeking an experienced Clinical Resource Nurse, licensed as an RN in California who will partner with our clinical leaders of our facilities in California. The ideal candidate must possess at least five years of experience as a Director of Nursing for Skilled Nursing Care facilities, have a strong understanding of CMS and CDPH regulatory requirements, possess the capacity to lead staff and be willing to travel amongst the assigned facilities on a frequent basis. The primary purpose of your job position is to coach the Directors of Nursing in your region toward clinical, cultural, and financial excellence through intentional leadership. You will guide the teams to care for their residents and staff not only to meet the regulatory requirements but assist to refine processes that help the teams become the best care givers they can be. This job will require constant presence in the facilities and will not be remote. Qualifications Education: · Graduate of accredited school of nursing, BSN or MSN preferred. GENERAL PURPOSE: The LVN Staff Nurse provides medical support services to the providers in order to ensure a smooth flow of patients through the medical clinic. MAIN RESPONSIBILITIES & DUTIES: 1) Assists patients calling and visiting the clinic a. Calls and escorts patients to exam area timely to promote continual flow of patients. b. Obtains vital signs, height and weight and documents in Electronic Health Record (EHR) per policy and procedure c. Collects and processes laboratory specimens per policy and procedure d. Enter patient with designated problems and data into i2iTracks per protocol e. Records in EHR timely per policy and procedure (PCMH) f. Maintains daily EHR worklist 2) Assists providers with patient health care support a. Performs diagnostic tests per protocol and/or at provider request b. Administers immunizations and medications per protocol c. Assists with calls to pharmacies per protocol and clinical messages to be returned in 48 hours (PCMH) d. Provides patient education and teaching at provider discretion g. Initiates Care Plans and updates EHR per Patient Centered Medical Home (PCMH) h. Initiates Advanced Directives and updates as appropriate per policy and procedure per PCMH e.
