JOB SUMMARY Assisting the Director of Nursing Services in the planning, organizing developing and directing of the day-to-day functions of the Nursing Department in accordance with current federal, state, and local standards, guidelines and regulations, and facility policies. Under supervision of a clinical leader, meets with the CNA Ladies and Gentlemen of the nursing department, on a regularly scheduled basis to assist in identifying and correcting problem areas, and/or the improvement of services. Recruit and conducts preliminary interviews and make recommendations for hiring to the clinical leadership and Administrator. Final interviews and approval are completed by the clinical leadership or Administrator. Provides assistance with orientation, coaching and mentoring to ensure the success of CNAs in the performance of their duties. Makes recommendations for counseling to the clinical leadership. Provides feedback to clinical leadership regarding CNA daily and annual performance evaluation feedback. Make daily rounds to ensure that nursing personnel are performing required duties, and to ensure appropriate procedures are being followed. Make physical rounds of all customers daily. Make rounds with physicians as necessary. Maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department. Plan, conduct and participate in In-service Education Programs. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. Coordinate resident care plan with the physical, occupational, respiratory, and speech therapy department to ensure proper care and treatment when clinically appropriate. Schedule and conduct regular conferences with the staff to discuss changes in resident's condition and problems or concerns regarding resident's care. Delegate the responsibility to daily review the documentation of dispensing controlled substances and narcotics. Reviews the records no less than weekly, or sooner if a discrepancy is noted, to ascertain that they are properly counted and documented from shift to shift. Assist in completing audits of sample EMR charting monthly to ascertain proper nursing charting and documentation of care given and directly confer with those employees who are not properly documenting, or delegates this responsibility. Review admissions/referrals and assist in interviewing/selecting residents for admission with the Administrator's approval. The MDS / Care Plan Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents in order to assure appropriate reimbursement for care and services provided within the Facility. Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category. Oversees the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission. He/she will integrate nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assesses and determines the health status and level of care of all new admissions. Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change. Communicates level of care for new resident to all disciplines. Coordinates interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames. Ensures completeness and thoroughness of documentation as mandated by federal, state and medical standards. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments. Our Caregivers are the keystone of our business, providing one on one care to seniors and their families struggling to keep their independence on a daily basis. Our Caregivers possess the virtues of compassion, care, dependability, professionalism, honesty, flexibility, problem solving, a positive attitude and love. From basic companion care (which includes, but is not limited to light housekeeping, sitter care, transportation, meal preparation, etc.) to more integral personal care (which includes, but is not limited to incontinence care, transferring, dressing and bathing, etc.), our Caregivers provide a wide range of services.
