Assist with mobility, therapeutic exercises, and active or passive range of motion as directed. Support patients with meal preparation, feeding, light housekeeping, laundry, and maintaining a clean, safe home environment. Monitor patient conditions and promptly report changes to the RN Case Manager or RN Supervisor. Perform delegated nursing or therapy procedures as directed by the supervising RN or therapist. Provide emotional support and companionship while promoting patients' comfort, dignity, and mental well-being. Complete accurate and timely electronic documentation, with at least 90% completed at the point of care and all visits documented the same day. Participate in mandatory in-services, complete annual education requirements, and attend team meetings as required. Participate in rotating weekends based on patient care needs. Home Health Aide (CNA) Qualifications: Current Minnesota Certified Nursing Assistant (CNA) certification. Ability to successfully complete Medicare competency requirements.
