Summary: Provides primary and/or specialty nursing and/or medical care in a variety of healthcare settings. Functions in collaboration with a physician according to a standard care arrangement and specialty board certification. Operates in accordance with Section 4723.431 of the Ohio Revised Code and rule adopted by the Board of Nursing for Advanced Practice Nurses, the national board certifying agency, and maintains a Standard Care Arrangement with a collaborating physician(s) on the medical staff. Performs health assessments and physical exams Identifies significant health and medical risk factors. Orders, interprets and evaluates tests and studies to assess clinical problems and health care needs. Develops a diagnosis by analyzing and synthesizing collected data. Independently and in collaboration with the physician, develops treatment plans along with the patient and significant others. Performs diagnostic or therapeutic interventions consistent with board certification, state law and organizational medical staff privileges. May prescribe and monitor medications as allowed by prescriptive authority through Ohio Board of Nursing and hospital policy. Develops patient education plan. Participates in medical team rounds; collaborates with nursing, medical and other healthcare team members regularly to ensure quality patient care. Performs admission history & physical, document progress notes, complete discharge summaries as required. Provides consultations and requests consults as directed and in accordance with hospital bylaws. Assists with and facilitates discharge planning. May serve as resource for staff/residents and clinical preceptor for APN students. Other duties as assigned. Responsibilities: 1. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Providing patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. 3. Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, O2 etc.) 4. Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned. 5. Assisting in dressing and undressing patients as assigned. 6. Works with Professional Relations Representatives (PRRs) to provide education on all available services. Provides care for any GIP patients in a hospital including daily visits, professional management, documentation and efficient discharge. Provides and manages direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up. Demonstrates clinical skills regarding case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships. Prioritizes, consults, refers, and collaborates with other professionals to promote quality and continuity of patient/family care. Educates patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process, and safety practices. Reports changes in the patient’s condition to appropriate members of the IDG or other health professionals. Attends patient deaths; prepares body for transport; oversees disposal of medications; and provides bereavement support. Completes documentation per policy and procedure. During times of emergencies (i.e. Hurricanes, etc.), the RN, Nights and Weekends may be required to report to work at a location designated by the company, to ensure continuity of services. This may include reporting to work ahead of your scheduled date/time due to planned lock down of unit, and staying overnight(s) based on duration of emergency. Perform other duties as assigned.
