Hospice Registered Nurse Case Manager

JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. The clinic serves general pediatrics as well as all pediatric subspecialties at main campus, with each subspecialty aligned to a designated pod and supported by a dedicated primary nursing team (LPNs and MAs/CTs). As a Medical Assistant on this team, you will play a vital role in creating a welcoming and supportive environment for patients, families, and visitors. You’ll work collaboratively with fellow staff to support medication management, patient monitoring, and a variety of administrative and clinical tasks. A caregiver in this position works rotating weekly shifts from 7:30 a.m. – 4:00 p.m., 8:00 a.m. – 4:30 p.m., 8:30 a.m. – 5:00 p.m. and 9:00 a.m. – 5:30 p.m. A caregiver who excels in this role will: Assist with performing screenings and treatments by gathering and updating information related to health maintenance, identifying medication changes, updating allergy information, collecting specimens, performing tests and documenting patient care. Monitor and communicate changes in patient condition. Administer specific medications under the direction of the provider. Maintain equipment, medical supplies and examination areas. Perform administrative/clerical duties, such as filing, reception, scheduling, data entry and patient registration.
