About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Connecticut shoreline (Groton-New Haven) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. Our Hospice Nursing Aides may assist patients' family members in caring for their terminally ill relatives by instructing them on how to go about performing basic health-care routines. Nursing Aides consult on a regular basis with their supervising registered nurses in terms of keeping hospice team members apprised of our patients' progression and the families' needs. Qualifications Must meet one of the following requirements. 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); 2) have one year experience within the last five years in direct client care in a home health or hospice setting. Must be registered and listed in good standing on the nurse aide registry in the state of employment. Must have basic demonstrated technology skills, including operation of a mobile device.
