Answer call lights promptly. Help check in all new residents and re-admissions, as well as assist in the transfer of residents to different rooms. Put resident's clothing away properly. Report any changes in resident's condition--e.g. eating habits, behavior, temperature, etc. to the charge nurse of the unit. Turn bedridden residents every two (2) hours or sooner as indicated. Wash, clean, and dry all incontinent residents. Assist the treatment nurse as assigned. Complete assignment report to charge nurse at the end of every shift. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Take vital signs and monthly weights (with assistance) and record results in EMR. Document the following in the EMR for all assigned residents: a) vital signs b) weight c) meal monitoring d) bowl and bladder e) restorative records Detect and report situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Assure that equipment is cleaned and prepared for the next shift. Assure that work schedules are followed. Maintain the confidentiality of resident information and honor his/her personal and property rights. Attend nursing department, Care Plan, and other meetings as directed by the DON or ADON. Attend and participate in in-service educational classes and on-the-job training programs as scheduled or as directed. Turn in Days Off and payroll special request forms in a timely manner. JOB SUMMARY Responsible to provide direct nursing care to the customer, and to supervise the day-to- day nursing activities performed by the nursing assistants. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the customer, as well as the customer’s response to the care. Complete required assessments upon the customer’s admission, transfer, and/or discharge. Receive telephone orders from physicians and record on the Physicians’ Order Form and P.O.S. Record new/changed diet orders. Forward information to the Dietary Department. Perform routine charting duties as required and in accordance with our established Charting and Documentation Policies and Procedures. Responsible for completion of admission, discharge, and transfer processes. Prepare and administer medications and treatments if appropriate as ordered by the physician. Review medication record for completeness of information, accuracy in the transcription of the physician’s order, and adherence to stop order policies. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, care of the dead/dying, etc., as required. Participate in the orientation of new customers/family members to the facility. Arrange for diagnostic and therapeutic services, as ordered by the physician. Notify the customer’s attending physician and family when there is a change in the customer’s conditions. Obtain sputum, urine and other lab tests as ordered. Take and record TPR’s, blood pressures, accuchecks, etc., as necessary. Contact the customer’s physician for: Nursing assessment of change of condition. Medication orders or charges. To inform of incidents. To report abnormal results of Lab and X-ray. Anything else pertinent to the customers’ care and well-being. Ensure the nurses’ notes and monthly summaries reflect the care plan and is being followed. Attend staff meetings, etc., as directed. Perform other related duties as assigned. Order and give all new medications and chart same when done using the EMR system. Direct charting in his/her shift and make monthly detailed evaluation of all resident charting so that charts reflect progress and condition of residents in the EMR system. Be responsible for well-being and nursing care of all residents assigned to his/her unit while on duty. Contact families regarding significant decline in resident's status or transfer to hospital documenting contact in the EMR. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the DON. Ensure that all medications and treatments are charted by the person administering the medication or completing the treatment on his/her assigned shift (EMR system). Prepare incident/accident reports, events and observations using the EMR system. Ensure that a copy of the medical records or an abstract of them are forwarded with the transfer of residents to a hospital or other health care facility. Detect and correct situations that have a high probability of causing accidents or injuries to residents and/or staff. Assure that work/assignment areas are clean and free of hazardous conditions, i.e. spills, excess carts, etc. and that equipment, tools, supplies, etc. are properly stored while working, as well as before leaving such areas for breaks, meal times, and end of the work day. Assure that established infection control and universal precaution practices are maintained when performing nursing procedures. Follow established safety precautions when performing tasks and when using equipment and supplies. Report all equipment malfunctions and breakdowns to the ADON as soon as possible and keep the ADON informed of supply needs and equipment needing replacement. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $115,000.00 to $130,000.00 on an annualized basis.
