Duties & Responsibilities Work in collaboration with physicians to provide patients with care, education, and support. Assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. May advise patients on health maintenance and disease prevention or provide case management. Coordinating and assigning duties to other health care personnel, such as licensed practical nurses, nursing aides, and patient care associates. Responsibilities include: (Detailed Task) Comply with procedures, regulations, and rules in maintaining medical records Monitor, record and report symptoms and changes in patients' conditions. Record patients' medical information and vital signs. Modify patient treatment plans as indicated by patients' responses and conditions. Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans. Monitor all aspects of patient care, including diet and physical activity. Direct and supervise less skilled nursing/health care personnel, or supervise a particular unit on one shift. Prepare patients for, and assist with, examinations and treatments. RN Field Nurse ( Weekends)

The Registered Nurse is responsible for the delivery of patient care services through coordination, implementation, ad supervision of field staff. The Registered Nurse Supervisor ensures that field staff follows the POC and delivers care accordingly. The Registered Nurse particopates in quality improvement activities within the agency, promoting overall compliance with State and Federal guidelines and professional standards of practice. Position qualifications: * Must be a registered nurse with a public health, home health care, or hospital background * Must have at least 2 years clinical experience Physical qualifications: * Must be able to hear and speak in a manner understood by most persons. * Must be able to travel to prospective patient's place of residence * Must be able to stoop and bend; must be able to lift and transfer patients.
