Your Role on the Team The position of Nurse Practitioner 2 in the Department of Surgery, requires a highly competent, independent yet team-oriented individual who will be immersed in the management and coordination of all inpatient care for the various division of department of surgery, including Trauma and Acute Care Surgery Service. Duties include and not limited to assessing health needs, planning and evaluating care of all patients on the general surgery services as well as patients in the surgical intensive care unit. The nurse practitioner will participate with inpatient sign out at beginning and end of shift. Follows up with the General Surgery teams/residents/attending physicians. Will be involved in trauma resuscitations and performing surgical consultations on potential general surgery patients. Responsibility for care and organization of procedures (including scheduling operations, interventional procedures, etc) and follow-up, obtain consults as needed, and discharge patients (including discharge summary, instructions, and medications) when appropriate. Triage phone calls. Responsible for the reviewing critical labs, follow-up of results with patient and medical staff, obtaining comprehensive medical histories, performing initial and periodic physical examinations/assessments, and exploring patient’s interpretation of health needs. Incumbent will identify and treat patients and manage treatment of hospitalized patients. The NP demonstrates an advanced level of medical knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation. Under the supervision of Chief of Trauma, Burns, and Critical Care Services and guided by the applicable policies and procedures, the Nurse Practitioner will participate in the comprehensive care of patients in the ambulatory or inpatient settings and provide multi-disciplinary care coordination, treatment, education, monitoring, and communication with patients and referring physicians. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. Certified Nursing Assistant (CNA) CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30° for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. JOB DESCRIPTION Job Summary Provides medical assistant support for Care Connections team. Responsible for contacting members following appointments/services rendered to ensure member understanding and navigation of the health care system, and coordination support for follow-up care. Also provides assistance for other administrative clinical support tasks. Contributes to overarching strategy to provide quality and cost-effective member care. Essential Job Duties • Coordinates care between members and providers after appointment/service delivery to support member navigation of the health care system, needs related to follow-up care and highest quality care/desired member health outcomes. • Communicates with Care Connections providers to clarify orders/requests and assists with escalation of issues to appropriate team(s) when unable to resolve independently. • Supports processing and follow-up for care management assignment and primary care provider referrals for high-risk members. • Escalates member issues related to prior authorizations for medications and durable medical equipment. • Communicates lab results and other exam findings with members and primary care providers. • Communicates with nurse practitioners on member health issues to ensure timely follow-up and response. • Supports care coordination and intervention by sending electronic messages/faxes to primary care providers. • Closes gaps in health care by preparing member lab result letters and coordinates follow-up on lab results. • Fulfills nurse practitioner supply orders and maintains adequate nursing supply inventory. • Performs administrative tasks such as telephone and email response, documentation follow-up and updates to member records. • Facilitates follow-up calls to members to ensure positive member experience/outcomes and ensure needs are met. • Assists with special projects under the direction of clinical leadership. Required Qualifications • At least 1 year of health care experience preferably in a family practice, home health, primary care, palliative care or hospice setting, or equivalent combination of relevant education and experience. • Graduate of an accredited medical assistant (MA) program. • Demonstrated understanding of patient care, medical terminology, coding procedures, reference tools and appropriate clinical pharmacology for medical assistant practice scope. • Excellent customer service skills. • Ability to work independently or as part of a team. • Ability and desire to meet or exceed Care Connections clinical integration and departmental metrics and conjunctively provide excellent customer service. • Strong attention to detail, ability to multi-task, prioritize, meet deadlines and demonstrate follow-through. • Utilizes critical-thinking skills to identify issues, problem-solve and demonstrate initiative. • Empathy for senior, disabled, income challenged and vulnerable populations. • Ability to build positive working relationships with peers and effectively manage conflict. • Strong verbal and written communication skills. • Microsoft Office suite/applicable software program(s) proficiency, and ability to learn new programs. • Ability to lift up to 50 lbs. Right at Home Long Beach is looking for an experienced CNA or LVN to provide hands-on, in-home training to our caregivers. This is a per-visit, hourly position — you'll be scheduled as needed to go into clients' homes and coach caregivers through the hands-on skills that are hardest to learn from a manual, with a particular focus on safe transfers and care for clients with limited mobility. The Caregiver Skills Trainer is responsible for creating extraordinary client and family experiences and ultimately trains our caregivers in the home. Provide one-on-one, in-home training to caregivers on safe transfer techniques, including Hoyer lift (mechanical/full body lift) transfers and gait belt transfers. Train caregivers on the care of bed-bound clients: safe repositioning and turning, use of draw sheets, skin and pressure injury (bedsore) prevention, and proper body alignment. Demonstrate and correct proper body mechanics to protect both caregiver and client during transfers, ambulation, and repositioning. • Coach caregivers on two-person transfers, sit-to-stand and pivot transfers, and wheelchair/bed-to-chair transfers. Train on personal care for clients with limited mobility: bathing, toileting/incontinence care, dressing, and oral hygiene in bed. • Review infection control and standard precautions (hand hygiene, PPE use, catheter and wound-adjacent care). Train on recognizing and reporting changes in condition — skin breakdown, signs of infection, falls risk, and changes in mental status. Observe caregivers performing skills in real time, provide constructive feedback, and document competency/training completion for the client's file. Communicate observations and concerns to the office and clinical team promptly. Model RAH's caregiving standards and represent the agency professionally in clients' homes.
