Description The (Per Diem) Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. 2. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. 3. Integrates relevant assessment and intervention skills in the delivery of nursing care. 4. Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. 5. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care and delegating to others as appropriate. 6. Educates patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease. 7. Communicates effectively and professionally with patients, family and all members of the Interdisciplinary Patient Care Team. 8. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience 1. Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP, AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. 2. Establishes and maintains a therapeutic relationship with the patient and family. 3. Uses evidence-based practices to increase understanding of patients perceptions of care. 4. Initiates service recovery for patients and family members who have concerns and escalates to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager or Patient Relations as needed. 5. Demonstrates caring, respect, compassion, empathy and active listening through dialogue, body language and actions. 6. Supports staff use of evidence-based practices to increase understanding of patients perceptions of care. 7. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety 1. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. 2. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. 3. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. 4. Engages in formal and informal peer and institutional review processes. 5. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. 6. Contributes to quality and safety practices and required compliance measures. 7. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. 8. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. 9. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations 1. Performs charge nurse duties as assigned. 2. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Shift Manager. 4. Demonstrates patient focused and cost effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. Professional Development 1. Contributes to the profession of nursing through participation in professional organizations. Qualifications Graduation from an accredited Nursing program. Bachelor's Degree in Nursing. (BSN) Associate's Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year. Any Associate's Degree in Nursing (ADN) RN with Nursing experience must be approved by the CNO. The Opportunity The Med Tech position provides support for our community’s residents with the essential day-to-day tasks they can no longer perform or need help with. Med Techs are responsible for administering daily medications. Duties include monitoring residents, reporting changes, and collecting samples. Description RN/Case Manager MSH Case Management PT Days The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. How you make a difference The Registered Nurse is responsible for assisting in the delivery of patient care through the nursing process of assessment, planning, implementation, and evaluation.
