Duties depend on specific client needs, and may change over time. Accompany the client to doctor appointments, assist with coordinating client care between different providers. Make recommendations to client on what care options may best meet their needs, to include skilled care, therapy, PT, nutrition, and personal care. Assist the client with understanding and implementing guidance from health care professionals. Assist with keeping client family informed. Maintain careful documentation of client case. If desired, be available to assist with homecare client in-home assessments Required Skills & Qualifications RN with current license to practice in Virginia Certification as CMC, CCM, or other Care Management certification, or the requisite experience to obtain Care Management certification. Previous experience working in Homecare, Assisted Living, Home Health, Skilled Nursing Facility highly desirable. Highly proficient communicator, comfortable with various communication methods including phone, email, Zoom, telehealth, etc. Reliable transportation and willing to travel to client homes and appointments.
