Position Overview Navigates and coordinates the care of patients across the continuum of care. Monitors and manages resources to transition patients to the appropriate level of care in a cost-effective fashion while ensuring quality outcomes and patient satisfaction. Assesses, plans, coordinates, monitors and evaluates the patients’ plan of care in collaboration with the healthcare team.
