Position Summary: The Licensed Practical Nurse (LPN) will be responsible for providing direct patient care under the supervision of a Registered Nurse (RN) and in accordance with the policies and procedures of Ditmas Park Dialysis Center. The LPN will play a critical role in the daily operations of the dialysis unit, ensuring the safety and well-being of our patients while maintaining a high standard of care. Key Responsibilities: Patient Care: Monitor vital signs. Prepare patients for dialysis treatment, including cannulation of access sites. Administer medications and IV fluids as prescribed. Monitor patients during dialysis treatment, addressing any complications or concerns. Provide patient education on dialysis care, diet, and medications. Assist in the development and implementation of individualized patient care plans. Equipment and Safety: Operate and maintain dialysis machines and equipment. Ensure the proper setup and breakdown of dialysis machines. Follow infection control and safety protocols to prevent cross-contamination. Conduct routine checks on equipment to ensure functionality and safety. Documentation: Maintain accurate and detailed patient records. Document patient progress, treatment plans, and any changes in condition. Ensure all documentation complies with regulatory and organizational standards. Team Collaboration: Work closely with RNs, nephrologists, dietitians, and other healthcare professionals to provide comprehensive patient care. Participate in team meetings and contribute to continuous improvement efforts. Assist in training and mentoring new staff members. Quality Improvement: Participate in quality improvement initiatives to enhance patient care and operational efficiency. Report any incidents, errors, or safety concerns promptly to the Nurse Manager. Qualifications: Education: Graduate of an accredited LPN program. Licensure: Current and valid LPN license in the state of New York. Experience: Previous experience in a dialysis or acute care setting preferred. Skills: Strong clinical and assessment skills. Excellent communication and interpersonal skills. Ability to work effectively in a team-oriented environment. Proficiency in electronic health records (EHR) systems. Certifications: Basic Life Support (BLS) certification required.
